Carry out patient care tasks and medical orders as delegated by physicians, APPs or other health care personnel following the Care Model Process.</span></p><p><span><strong>POSITION RESPONSIBILITIES:</strong></span></p><ul><li><span>Maintains confidentiality of sensitive information.</span></li><li><span>Conducts pre-visit planning, visit process, and other Care Model Process workflows.</span></li><li><span>Performs accurate and timely charting to support patient care and service.</span></li><li><span>Obtain health history information.
