With a flexible schedule and independence, you can make it possible for patients to receive the important care they need where they’re most comfortable – in their homes</p>\n<p> </p>\n<p>The RN Case Manager - Hospice uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient’s place of residence. The RN Case Manager - Hospice develops and oversees the hospice plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
