Initiate a nursing assessment of patient&rsquo;s condition upon admission.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">b. Recognize and assess initial alterations to the patient&rsquo;s body systems and communicate with clinical team.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">c. Organize and record assessment data for accuracy, completeness, and confidentiality.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">d. Assess, stage, and measure wounds using photographic technology accurately and in accordance with policy and procedure.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">e. Identify patient care situations that require intervention, implement nursing actions, and evaluate responses.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">f. Identify educational and discharge needs to patients and their families and implement a teaching plan of care.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Planning:</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">a. Collaborate with Staff RN Case Manager to identify the patient&#39;s present and potential problems from the assessment.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">b. Implement a teaching plan in collaboration with Staff RN/Case Manager.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">c. Prioritize problems according to impact on health status.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">d. Formulate with Staff RN/Case Manager the desired outcomes that are specific and measurable within a certain time frame and consistent with other health provider&#39;s expectations.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">e. Assess home health care needs, complete a support system evaluation, and assess patient teaching needs from admission to discharge. Communicate information to Staff RN/Case Manager.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Implementation:</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">a. Participate in implementing planned changes and activities to improve care.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">b. Hold self-accountable for the delivery of quality nursing care.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">c. Promote harmonious relationships and favorable attitudes among team members.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">d. Implement wound care and HBO protocols when clinically indicated.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">e. Individualize care according to age specific population.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">Evaluation:</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">a. Evaluate achievement or lack of achievement of desired outcomes.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">b. Collaborate with providers and the clinical team to ensure plans of care are appropriate, revising as necessary, and continually evaluating until desired outcomes are realized.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">c. Complete 4-week reviews in a timely manner and ensure all recommendations to the plan are activated in the provider orders.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">General:</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">a. Function as an intake/discharge nurse during clinic times.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">b. Foster continuity of care between the Wound Care Department and caretaker (home, hospital, home health, etc.).</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">c. Prepare clinic records to enable accurate and complete updates to the physician.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">d. Assist others within the clinic.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">e. Maintain open, professional communication with patients, physicians, and staff.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">f. Safely operate all clinical equipment.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">g. Utilize appropriate infection control procedures.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">h. Maintain skills and knowledge through training and continuing education. Complete mandatory education requirements. Attend staff meetings and in-services.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\">i.
