We even offer pet insurance for your four-legged family members.  </li>\n</ul>\n<ul>\n <li>Early access to earned wages with Daily Pay, tuition reimbursement to help further your career and adoption assistance to help you grow your family.   </li>\n</ul>\n<p>With a collective goal to champion a culture of belonging where everyone feels valued and respected, we honor the ways people are unique and embrace what brings us together.   </p>\n<p>And, we believe equipping you with support and development opportunities is a vital part of delivering an exceptional employment experience. </p>\n<p>Find a fulfilling career and make a difference with UnityPoint Health.</p>\n<h2>Responsibilities</h2>\n<p><strong>Care Coordination</strong> </p>\n<ul>\n <li>Promotes positive customer relations in dealing with patients, visitors, and other employees. </li>\n <li>Interacts with clients to promote a full range of health services; determines the benefit of each health program after assessing clients' willingness to enroll in available programs. </li>\n <li>Provide cultural and linguistic health education information and current resource materials to individuals and families. </li>\n <li>Utilize a holistic approach (considering physical, psych/social, spiritual, educational, safety and related criteria) appropriate to the age of the clients served in the assigned service area. </li>\n <li>Refers families to clinical, social, and financial support using an established algorithm to assess patient and family needs. </li>\n <li>Assist patients with accessing community-based resources by supporting them with applications, transportation needs and follow up on any referrals made to ensure patient utilizes identified community based resources. </li>\n <li>Advocate for individual and community health. </li>\n <li>Enhance patient’s ability to effectively communicate with healthcare providers. </li>\n <li>Works with staff and providers to create a care plan and communicates with them regarding the status of referrals and outreach efforts. </li>\n</ul>\n<p> </p>\n<p><strong>Administrative Duties</strong> </p>\n<ul>\n <li>Maintain documentation of patients served and support provided. </li>\n <li>Complete data entry to track patient outcomes, identify challenges and recognize success. </li>\n <li>Maintain up -to -date information about community-based resources in order to make appropriate referrals. </li>\n <li>Identifies trends in barriers experienced by families when seeking care and provides data to team members.  </li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education: </strong></p>\n<p>High school diploma or equivalent. </p>\n<p> </p>\n<p><strong>Experience: </strong></p>\n<p>Strong understanding of community based resources. </p>\n<ul>\n <li>Experience as a direct care worker </li>\n</ul>\n<ul>\n <li>Familiar with the VNS Community Resource Directory and/or 211  </li>\n</ul>",
