Join us!</p><p><br></p><p>HealthFirst Family Care Center, a Certified <strong>Great Place to Work</strong> and wonderful place to make a difference, is seeking a Psychiatric Mental Health Nurse Practitioner to join our team of mission-minded healthcare professionals.</p><p><br></p><p>The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides comprehensive psychiatric evaluation, diagnosis, medication management, and Medication-Assisted Treatment (MAT) services to patients of HealthFirst in accordance with professional standards, evidence-based practices, and quality performance expectations.</p><p><br></p><p>The PMHNP works collaboratively within an integrated care model alongside HealthFirst’s Behavioral Health, Primary Care, and Substance Use Disorder (SUD) teams to support patients experiencing mental health and co-occurring substance use disorders. This role includes assessing, prescribing, and managing psychiatric and MAT medications, supporting recovery-oriented care, and promoting whole-person wellness for adult and youth patients.</p><p><br></p><p><strong>DUTIES and RESPONSIBILITIES:</strong></p><ul><li>Provide psychiatric services to clients in a variety of clinical settings during scheduled hours. The scope of services includes psychiatric evaluations and medication management services.</li><li>Assess, diagnose, and treat adults, adolescents, and children experiencing symptoms of a behavioral health disorders.</li><li>Examine or order laboratory or diagnostic tests for clients to provide information on general physical condition and mental disorders.</li><li>Design/participate in the development of individualized treatment plans, incorporating best and leading practices into treatment interventions.</li><li>Responsible for timely and accurate completion of medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries.</li><li>Responsible for making independent decisions based on protocols developed utilizing national guidelines for treatment interventions.</li><li>Collaborate with counselors, social workers, or other treatment team members to coordinate client care.</li><li>Prescribe, recommend, or discontinue drugs and treatments to treat mental, emotional, or behavioral disorders in accordance with the Collaborative Practice Agreement and the state’s Nurse Practice Act.</li><li>Collaborate with guardians, relatives, and significant others on client’s conditions and treatment when clinically appropriate.</li><li>Teach, counsel, and provide emotional support, as it is an integral part of the provider’s role in providing the continuum of care for HealthFirst’s Clients.</li><li>Assume responsibility for ongoing health maintenance by initiating pro-active health care needs of patients to carry out their prescribed medical regime and utilize resources to maintain an optimal level of health and wellness.</li><li>Participate in Quality Assurance and Performance Improvement activities.</li><li>Maintain patient care and practice in accordance with federal, state, and agency guidelines.</li><li>Establish, implement, and evaluate research and standards of nursing practice to assure quality care.</li><li>Actively participate in all agency and department education, training, and in-services to confirm understanding of new information or changes to established clinical policies and procedures</li></ul><p><br></p><p>HealthFirst Family Care Center, Inc. is proud to be an equal-opportunity employer.
