Program Description: NYC Treat to Quit is a program of Public Health Solutions (PHS) and primarily funded by the New York State Department of Health (NYSDOH). The mission of NYC Treat to Quit is to advance health equity by strengthening medical and behavioral health systems, expanding access to evidence-based tobacco dependence treatment, and empowering communities disproportionately impacted by tobacco-related harm. Specifically, the objectives of PHS’s current five-year contract with NYSDOH are to 1) directly advance health equity by reducing tobacco-related health disparities and inequities in the catchment area 2) work with medical and behavioral health systems to adopt the Clinical Practice Guideline for Treating Tobacco Use and Dependence (Public Health Service(PHS) Guideline) 3) enable grantees to become well-versed in the most up-to-date medication treatment protocols for TUD 4) educate and advocate for local and state laws, regulations and voluntary policies to reduce tobacco exposure 5) support statewide efforts on data collection, research development and distribution of resources to enhance public awareness, engagement and action on tobacco use, e-cigarettes and cessation. Summary of Position: NYC Treat to Quit at Public Health Solutions seeks a Health Systems Partnerships Coordinator who will be responsible for building and sustaining relationships with cessation providers, healthcare and community-based organizations, and local coalitions. The coordinator will promote cross-sector collaboration and support access to services to populations disproportionately impacted by tobacco use. The Health Systems Partnerships Coordinator will work closely with the NYC Treat to Quit team and will receive direction from the Senior Health Systems Manager under the guidance of the Program Director. This is a grant-funded position ending April 30, 2031. Specifically, the Health Systems Partnerships Coordinator will: Understand and implement the NYS Department of Health Tobacco Control Program’s strategic plan as well as support the development and implementation of PHS’ tobacco control strategic plan. Collaborate with the NYC Treat to Quit staff to achieve region specific work plan deliverables. Systems Collaboration and Engagement When needed, represent the program in healthcare system and partner meetings, events, and workshops to support tobacco cessation initiatives and promote tobacco-free living through coordinated partnerships Assist in coordinating healthcare partner and stakeholder engagement listening sessions to gather feedback and inform implementation and partnership strategies Participate in and document healthcare entity engagement activities such as provider meetings, health system workshops, and partner convenings Partnership Development and Coordination Contribute to developing and maintaining partnerships with healthcare systems, public health partners and community health organizations Assist in identifying emerging community needs, barriers to cessation access and opportunities for expanded collaboration among priority populations Keep track of cessation resources, and promote access to cessation resources among populations disproportionately impacted by tobacco (LGBTQIA+, Asian American, low-income, menthol users) Identify, develop, and strengthen opportunities for referral pathways to tobacco cessation services, including coordination between healthcare organizations, healthcare providers, behavioral health systems and cessation resources such as the NYS Quitline and local treatment programs Assist in tracking partnership engagement activities, cohort participation and outreach outcomes to support program evaluation and continuous quality improvement Technical Assistance and Compliance Maintain and regularly update a directory of cessation resources, referral partners, and community-based services across NYC Accurately and completely report activities in the required formats. This includes the state program reporting systems and organization reporting systems. State Coordination and Responsibilities Attend and participate in mandatory local, regional, and state meetings and trainings. Anticipated 5 times per contract year. Actively participate in state program workgroup meetings. Along with team members, implement a robust health systems engagement strategy that aligns with organizational goals. Participate in the yearly data collection project assigned by the state program. Remain abreast of current on tobacco control issues and all other duties as assigned Qualifications and Experience: Bachelor’s degree or higher in a related field. At least 2 years of experience working with and for health care organizations, community-based organizations and demonstrated organizing skills. Demonstrated experience with policy and community health systems interventions, health systems engagement and partnership building Knowledge of tobacco cessation best practices, treatment approaches and emerging tobacco control issues a plus. Excellent computer, oral and written communication and interpersonal skills. Desired skills in coordinating and facilitating engagement initiatives with healthcare entities and partner organizations. Ability to engage with healthcare partners during non-traditional work times which can include evenings, weekends, and willingness to have a flexible schedule. Must have excellent organizational skills and the ability to coordinate multiple tasks, projects, and assignments with competing priorities. Must be able to work independently as well as be a team member. Familiarity with New York City communities and neighborhoods strongly preferred. Must be able to travel in between NYC boroughs, across NYS for participation in mandatory state meetings and when eligible across the nation for participation in trainings and conferences. This individual must reside in the tri-state area (NY, NJ, CT) by their confirmed start date. Reports To: Director of Tobacco Control Alliance and Senior Manager Health Systems.
