    "description": "<p><span>Duties and Responsibilities:</span></p>\n<ul>\n<li><br><span>Is responsible for making clinical decisions, exercising judgement to ensure best possible care is provided within available resources.</span></li>\n<li><span> </span><span> Provides counseling services, with particular regard to individual and family issues and decisions that impact health of the patient and family. Works within primary care model of brief therapy, ensuring seamless transition to community resources as appropriate.</span></li>\n<li><br><span>Is knowledgeable of the community and the resources available, especially in the areas of crisis services, mental health, and long-term care.</span></li>\n<li><br><span>Assists the referring physician/health care providers with referrals to secondary and tertiary health care centers.</span></li>\n<li><br><span>Knowledge and skill in group work including cognitive behavior therapy and mind over mood.</span></li>\n<li><br><span>Enthusiastic, passionate, and self-driven who will participate in data management and measurement of quality improvement indicators of patient outcomes.</span></li>\n<li><br><span>Communicates results of measurements that are easy to use and understand, looking for patterns that could assist with improvements.</span></li>\n<li><br><span>Acts as advocate for patients and their families in matters affecting health.</span></li>\n<li><br><span>Liaises with physicians and other team members as required.</span></li>\n<li><br><span>Teaches family medicine residents and other health care professional students on the role of the social worker and psycho-social issues facing patients and families.</span></li>\n<li><br><span>Effectively uses the social work standards of practice and the therapy process in assessing, planning, implementing, and evaluating the care of the patient.</span></li>\n<li><br><span>Maintains own continuing education to enhance professional knowledge and growth.</span></li>\n<li><br><span>Participates and collaborates with other family health team members and health care providers in provision of patient care.</span></li>\n<li><br><span>Acts as a professional and knowledgeable role model or mentor to colleagues and students.</span></li>\n<li><br><span>Maintains confidentiality in the workplace within the community.</span></li>\n</ul>\n<p><span>Education/Experience Requirements:</span></p>\n<ol>\n<li><br><span>Master's Degree preferred</span></li>\n<li><br><span>Registered with the Louisiana State Board of Social Work Examiners</span></li>\n<li><br><span>Previous related patient care experience, specifically in a primary care setting preferred</span></li>\n<li><br><span>Minimum 2 years experience preferred</span></li>\n<li><br><span>Experience in solution focused brief therapy, cognitive behavior therapy, motivational interviewing, and mindfulness</span></li>\n<li><br><span>Ability to operate with highest level of confidentiality and discretion</span></li>\n<li><br><span>Strong conflict resolution</span></li>\n<li><br><span>Exceptional oral, written, and presentation skills</span></li>\n<li><br><span>Strong interpersonal skills</span></li>\n<li><br><span>Group facilitation skills</span></li>\n<li><br><span>In depth knowledge of chronic illness care, including both clinical and supportive care</span></li>\n<li><br><span>In depth knowledge of the local health program delivery and community-based services</span></li>\n<li><br><span>Able to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patient, family, and provider population</span></li>\n<li><br><span>Ability to work effectively with all members of the health care team</span></li>\n<li><br><span>Ability to prioritize, manage time effectively and be flexible in a very active work environment</span></li>\n</ol>",
