The Biweekly deduction from employee paycheck is $40.00 for Employee Only, $95.00 Employee plus Children, $120.00 Employee plus Spouse, and $140.00 Employee plus family. &nbsp;</strong></p><p><br></p><p>CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening.</p><p><br></p><p><strong>JOB SUMMARY: </strong>&nbsp;Provides direct oversight of diabetes program operations. The Diabetes Program Coordinator, in conjunction with clinic and tribal leadership, develops and implements quality diabetes prevention and management initiatives, health education and community support efforts. The Diabetes Program Coordinator will assist grant management teams as needed for data and reporting measures.<br> <br>&nbsp;</p><p><strong>ESSENTIAL FUNCTIONS:</strong></p><p>· Develops, coordinates, and provides a comprehensive health management program for diabetic patients.</p><p>· Develops, coordinates, and provides a comprehensive preventive health program for pre-diabetic patients.</p><p>· Instructs diabetic education courses, cooking demonstrations, and performs nutrition evaluation.&nbsp;</p><p>· Responsible for maintaining American Diabetes Association (ADA) DSME recognition and participating in program reporting, evaluation, and audit.</p><p>· Aides grant management teams in the completion of SDPI (and other related grant) reporting.</p><p>· Maintains collaborative working relationship with clinic leadership and provider teams.</p><p>· Develops community outreach events, with support of clinic and tribal leadership.</p><p>· Effectively communicates will team members</p><p>· Responsible for the direct management and evaluation of all diabetes program staff.<br> <br>&nbsp;</p><p><strong>ADDITIONAL RESPONSIBILITIES:</strong></p><p>· Perform other duties and projects as required by management.</p><p>&nbsp;<br>&nbsp;</p><p><strong>MINIMUM QUALIFICATIONS FOR CONSIDERATION:</strong></p><p>· Licensed Dietitian in the State of Oklahoma.</p><p>· Bachelor’s degree in food and nutrition, dibetics, or a closely related field.</p><p>· Minimum two (2) years working with a diabetic population.</p><p>· Certified Diabetes Care and Education Specialist (CDCES) or willingness to obtain certification within 18 months of accepting employment.<br> <br>&nbsp;</p><p><strong>KNOWLEDGE, SKILLS, AND ABILITIES:</strong></p><p>· Prior experience in nutrition/ health services that equips the applicant with the specialized knowledge, skills, and abilities to successfully perform the duties of this position.</p><p>· Excellent written and verbal communication skills.</p><p>· Ability to maintain strict confidentiality regarding all patient encounters.</p><p><strong>Physical Requirements:</strong>&nbsp;</p><p>· Sitting and standing for prolonged periods of time; sitting behind a computer for prolonged periods of time; occasional bending, stooping, kneeling, squatting, and lifting.<br> <br>&nbsp;</p><p><strong>ENVIRONMENTAL CONDITIONS:&nbsp;</strong></p><p>· General clinical environment with frequent exposure to chemicals, bloodborne pathogens and patient body fluids.&nbsp;</p><p>&nbsp;<br>&nbsp;</p><p><strong>Note:</strong> The above statements are intended to describe the general nature and level of work performed by an employee in this position.
