Works collaboratively with other providers to facilitate client wellness. Provides remote, off work hours telephone triage to staff calls from OPWDD &amp; OASAS residential group homes. Maintains all applicable confidentiality and ethical standards of relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.&nbsp;</p><p><br></p><p><u><strong>Essential Duties and Responsibilities</strong></u></p><ul><li>Remote position, on call overnights, weekends and holidays</li><li>Evaluates the comprehensive health assessment of clients.</li><li>Identifies immediate health care needs.</li><li>Makes appropriate referrals and recommendations based on outcome of triage logic</li><li>Collaborates with appropriate Agency staff, and related community organizations to assure coordination of client care and services if needed.</li><li>Completes required electronic patient record documentation in real time.</li><li>Patient Assessment and Triage: Respond to urgent calls, assess symptoms, and determine the appropriate care over the phone.</li><li>Provide staff education and care instructions within the RN scope of practice</li><li>Document staff education accurately and in a timely manner in the EMR.</li><li>Demonstrates effective communication with staff to assure appropriate understanding of treatment requirements and objectives of those actions.</li><li>Strong clinical assessment and critical thinking skills.</li><li>Excellent verbal and written communication.</li><li>Strong interpersonal and customer service skills.</li><li>Extensive knowledge of anatomy, physiology and nursing care standards</li><li>Analytical skills to assess patient condition</li><li>Effective oral communication skills to interview staff and explain next steps as well as to pass information on to other members of the care team</li><li>Ability to work with diverse and vulnerable patient populations.</li><li>Excellent organizational, computer, and documentation skills.</li><li>Possess and maintain accurate knowledge of all regulatory and practice regulations and treatment protocols with special attention to HIPAA requirements</li><li>Always demonstrates the highest standards for ethical and professional conduct.&nbsp;</li><li>Proficiency with computer-based applications to independently complete work and add/use data</li><li>Demonstrate a thorough knowledge of services provided both by the Agency and by external service providers and maintain contacts as needed to efficiently access care for clients.&nbsp;</li><li>Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan.
