Our team of doctors, nurses, therapists, social workers, drivers and aides work together to provide all-inclusive care and help retain independence.</p>\n<p> </p>\n<p>PACE (Program of All-Inclusive Care for the Elderly) provides high-quality comprehensive home and community-based healthcare services to participants, always anticipating problems to help avoid hospitalization and prevent premature nursing home placement. </p>\n<p> </p>\n<p>The PACE Physician is responsible for providing medical oversight and clinical leadership for the care of elderly patients within our program in Sacramento.
