          "text": "<p>&#xa0;</p><ul><li><strong>Assess and Support Patients</strong>: Assist patients with overcoming physical, cognitive, and emotional barriers to maximize their independence in daily activities.</li><li><strong>In-Home Assessments</strong>: Conduct comprehensive in-home assessments to evaluate the physical, cognitive, and psychosocial abilities of clients.</li><li><strong>Collaborative Care Plans</strong>: Develop individualized care plans with direct input from clients and their families and to provide input and direction on the frequency and duration of needed OT interventions to meet client goals.</li><li><strong>Interdisciplinary Collaboration</strong>: Work closely with physicians, nurses, social workers, and other professionals to ensure the best outcomes for clients.</li><li><strong>Adaptive Equipment &amp; Safety</strong>: Assess and train clients and families on the use of adaptive/assistive equipment, safety equipment, mobility aids, and environmental modifications.</li><li><strong>Ongoing Monitoring</strong>: Reassess patients’ progress and adjust care plans to ensure optimal outcomes.</li><li><strong>Documentation</strong>: Maintain accurate documentation, including assessments, treatment plans, progress notes, and discharge summaries.</li><li><strong>Continuous Education</strong>: Participate in ongoing professional development and training.</li><li>Our clinicians will consistently demonstrate compassionate, caring and respectful behvariour in all interactions with client’s, healthcare partners, colleagues, management staff, and the public, reflecting the core values of Spectrum Health Care.</li></ul>",
