We’ll celebrate milestones and acknowledge the value of spirituality in healing.</p> \n<p>We’re proud of what we know, which includes how much there is to learn.</p> \n<p>&nbsp;</p> \n<p><strong>Your day.</strong></p> \n<p>As a Care Manager you need to know how to:</p> \n<ul> \n <li>Coordinates the care and services for patients identified as needing assistance or meeting Care Management criteria.&nbsp; Collects in-depth information about a patient’s medical, functional, and social condition, to identify individual needs in order to develop a plan to meet those needs. &nbsp; Works with patients and family/caregivers to determine specific goals and actions based on assessment.&nbsp; Coordinates discharge planning.&nbsp; Executes specific interventions to meet established goals.&nbsp; Organizes, integrates and coordinates the necessary resources to accomplish the goals and plan.</li> \n <li>Assesses the patient’s prior level of functioning, access to and/or use of community resources and available support systems.&nbsp; May&nbsp; conduct psychosocial needs assessments and interventions based on site guidelines, licensure, and scope of practice.&nbsp; May support behavioral health processes based on site guidelines.</li> \n <li>Assists the care team in developing a plan of care which includes, but is not limited to: assuring appropriateness of services and care setting, assuring individualized support and education, determining the need for continued services, planning for discharge, and identifying and connecting patients/families with available community resources if needed.&nbsp; Collaborates with Physicians and other members of the health care team on the patient’s behalf.&nbsp;&nbsp;</li> \n <li>Identifies appropriate admission and continued stay issues. Enhances the quality of patient care through effective and efficient use of resources.&nbsp; Helps to identify strategies for reducing length of stay and appropriate utilization of services.&nbsp; Utilizes criteria including clinical pathway data and implements strategies to resolve controllable variances.&nbsp;&nbsp;</li> \n <li>Attends, facilitates and participates in rounds and case conferences.&nbsp; Advocates for patient rights.&nbsp;</li> \n</ul> \n<p>&nbsp;</p> \n<p><strong>Your experience.</strong></p> \n<p>We hire people, not resumes. But we also expect excellence, which is why we require:</p> \n<ul> \n <li>Social Worker: Master degree in Social Work from and accredited program is required.</li> \n <li>OR</li> \n <li>Bachelor’s Degree in Nursing (BSN)</li> \n</ul> \n<p>Required:</p> \n<ul> \n <li>RN:&nbsp; Current valid State RN License is required.</li> \n <li>MSW:&nbsp;</li> \n <li>Colorado:&nbsp; Current valid&nbsp; LCSW license or</li> \n <li>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;SWC or LSW required at hire with&nbsp;</li> \n <li>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;LCSW required&nbsp; within 3 years of employment.&nbsp;</li> \n</ul> \n<p>Required:</p> \n<ul> \n <li>Three (3) experience as a Registered Nurse in an acute care setting or Social Worker in a community or acute care setting.&nbsp;&nbsp;</li> \n <li>Three (3) years experience as a Care Manager in an acute care setting.&nbsp;&nbsp;</li> \n</ul> \n<p>Preferred:</p> \n<ul> \n <li>At least five (5) years of care management in an acute care setting</li> \n <li>Care Management Certification is preferred</li> \n</ul> \n<p>&nbsp;</p> \n<p><strong>Your next move.</strong></p> \n<p>Now that you know more about being a Care Manager ­ on our team we hope you’ll join us.
