About the Role:
The RN, Hospice plays a critical role in providing compassionate, patient-centered care to individuals facing life-limiting illnesses. This position focuses on managing and coordinating comprehensive hospice care plans that address physical, emotional, and spiritual needs, ensuring comfort and dignity for patients in their final stages of life. The RN will collaborate closely with interdisciplinary teams, including physicians, social workers, and chaplains, to deliver holistic support tailored to each patient and their family. Additionally, the role involves educating patients and families about disease progression, symptom management, and available resources to empower informed decision-making. Ultimately, the RN, Hospice serves as a vital advocate and caregiver, enhancing quality of life during a profoundly sensitive time.
Minimum Qualifications:
Current and valid Registered Nurse (RN) license in the state of Oklahoma. Basic Life Support (BLS) certification. Strong knowledge of hospice care principles, symptom management, and pain control. Excellent communication and interpersonal skills to effectively interact with patients, families, and multidisciplinary teams. Ability to work independently in a community-based setting. Preferred Qualifications:
Experience working with diverse patient populations and understanding of cultural sensitivities in end-of-life care. Familiarity with electronic medical records (EMR) systems and hospice-specific documentation software. Responsibilities:
Conduct thorough patient assessments to evaluate physical symptoms, psychosocial needs, and overall well-being. Develop, implement, and regularly update individualized hospice care plans in collaboration with the interdisciplinary team. Administer medications and treatments as prescribed, monitoring patient responses and adjusting care as necessary. Provide education and emotional support to patients and their families regarding disease trajectory, symptom management, and end-of-life care options. Coordinate with healthcare providers, community resources, and support services to ensure seamless care delivery. Document all patient interactions, care plans, and clinical observations accurately and timely in compliance with regulatory standards. Participate in regular team meetings and case conferences to discuss patient progress and modify care strategies. Advocate for patient comfort and dignity, addressing any ethical or cultural considerations in care. Respond promptly to patient and family concerns, providing crisis intervention and support as needed. Skills:
