This role plays a critical part in ensuring safe transitions of care, preventing readmissions, and strengthening continuity with our Primary Care physicians.</p>\n<p>You will review discharge information, reconcile medications, and schedule timely follow-up appointments across <strong>16 Primary Care offices</strong>.
