In a home environment, we learn basic trust and a sense of self, culture, heritage and how to live with others. It is through this experience that we develop the skills to practice the self-regulation necessary to transcend into productive adults. It is within this culture that The Foundling's DD Division provides persons with developmental disabilities assistance to reach their maximum potential.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Responsible for providing nursing care and coordinating medical services for individuals in assigned IRA sites.</li>\n <li>Assessment of the medical needs of the individuals, plans intervention, and evaluates outcome of care and performs treatments and special medical care as mandated.</li>\n <li>Coordinates with other medical providers regarding care management.</li>\n <li>Responsible for monitoring medical care of individuals.</li>\n <li>Provide medical consultation regarding individual medical needs through 24 x 7 telephone tri-age</li>\n <li>Provide telephone triage to agency's staff</li>\n <li>Provides or assists all activities of daily living for individuals. i.e. adequate intake of food and fluids, bathing, dressing, bowel and bladder care/toileting, oral/nasal hygiene, skin care and grooming</li>\n <li>Administers medication</li>\n <li>Provides AMAP certifications and recertification classes for IRA staff.</li>\n <li>Monitors and provide supervision to all AMAP certified staff.</li>\n <li>Provides supervision of nutrition issues including assisting staff with menu preparation. Coordinates with dietician.</li>\n <li>Coordinates medical services with Residence, Schools, and Day Services.</li>\n <li>Attends in-service to maintain current knowledge and trends in developmentally disabilities and medical care.</li>\n <li>Implements plan of care as outlined in each individuals medical profile including positioning, play, feeding techniques, behavior modification and attention to ADL skills.</li>\n <li>Provides in-service training on medical issues as required.</li>\n <li>Record monthly individual progress notes for each individuals</li>\n <li>Complete job related tasks as needed</li>\n</ul>\n<p> </p>\n<p><strong>The New York Foundling is committed to attracting and retaining a diverse employee population, the Foundling will honor your experiences, perspectives and unique identity.
