Description

Child and Family Guidance Center is hiring an   ACT Team Lead- Qualified Mental Health Professional.   The position is a hybrid lead position supporting the ACT Director in North Texas clinics and community. Job Summary: Responsible for providing rehabilitation skills training for assigned consumers in the Assertive Community Treatment (ACT) program. Provides direction, guidance, training and leads a team of ACT QMHP’s in the performance of their job duties. Team Leader will lead daily staffing of cases for the team as needed. Team Leader will provide in the community shadowing, training and support of all ACT QMHP’s. Job requirements total a minimum of 100 direct client contact hours (400 units) to be provided each month with the goal of delivering 40 units per client monthly. Team Leader is required to work a rotating on-call capacity, to include weekends and non-business hours. Provides rehabilitative services designed to assist individuals in achieving an optimal quality of life. The goals of rehabilitation services include Development/enhancement of community living skills, increased management of mental health symptoms (symptom management), increased knowledge about medication, supported employment skills training, decreased psychiatric hospitalizations, and increased community tenure. Essential Duties & Responsibilities: Adhere to all current city, state and federal laws, rules and regulations, as well as CFGC Employee Handbook and any pertaining ethical standards.  This includes the safeguarding of confidential healthcare information and compliance with the Health Insurance Portability & Accountability Act (HIPAA). Required to provide curriculum-based rehabilitative services with consumers both individually and in groups. Independently develop and implement rehabilitative services, which are based on consumer desires, medical necessity, and clinical judgment. Delivery of rehabilitation services in accordance with recovery plans created collaboratively with clients and documentation of progress. Provide services according to the efficiency Benchmark standards set by agency. Transports consumers to and from appointments, as needed. Provide and maintain a valid driver’s license and copy of current auto liability insurance. This position requires approximately 85% local travel, and employees are responsible for providing their own transportation. Maintain all case records (notes, treatment plans, and uniform assessments) in accordance with agency standards. Document with the understanding that all clinical notations are considered legal documents and can be used in a court of law. All documentation should be true and accurate and under no circumstance falsified in any manner. Must have a working phone to provide services and be eligible for phone reimbursement Attend all Team, Individual, and Training Meetings as required and scheduled by management. Keep therapeutic skills updated by utilizing professional literature and training events, when required. Required to participate in on-going staff development. Maintain professional certification and/or licensure, when applicable. Prepare and present assigned cases for review by supervisor, as required. Immediately contact your supervisor if you suspect a consumer has been a victim of abuse or neglect. Immediately contact your supervisor if you have been called to testify or be involved in legal proceedings. Professionally represent self within community of involvement, understanding that you are a representative of a larger entity.  Ensure community is aware of all services offered within the agency. Provide consultation to vendor agencies with whom our agency contracts, as required. Will maintain cooperative working relationships with all agency staff and community partners. Coordinates client care and provides direct client care in the event of caseworker absence. 50% of time is spent in direct client care. Provides therapy and treatment services to ACT clients as needed. Monitors contact documentation to ensure that it is clinically appropriate and submitted in a timely manner. Responsible for intake interviews with potential consumers/family members for the purpose of preparing and submitting requests for authorization for ACT services. Prepares other clinical documentation; psycho-social assessment, reports to the court, mental illness warrants, etc., as needed. Job Qualifications: Knowledge:   General/basic understanding of diagnosis and treatment of clinical disorders and life adjustment problems.  General/basic knowledge of treatment documentation, confidentiality and ethical guidelines for practice.  Knowledge/awareness of service agencies works in a community setting; Knowledge of medications used in the treatment of mental illness. Education:   Must possess a minimum of a bachelor’s degree in rehab counseling, psychology, social work, or related field as specified in the Texas Administrative Code for QMHP qualifications. Skills : Should possess basic therapeutic skills, oral and written communication skills, and organizational skills. Must be goal oriented, motivated, and capable of working independently and using good judgment. Must possess a strong understanding and ability to work with diverse populations including various cultures, racial groups, and socio-economic levels. Must have an intermediate working knowledge of Microsoft Office Products (Outlook, Excel, Word, and PowerPoint). Ability to relate to consumers, employees, and the general public in a professional manner. Ability to follow verbal and written instructions. Must be able to prioritize. Ability to adhere to articulated policy and procedures. Ability to provide home visits as required. Experience:   Two (2) years of experience needed in a community setting with knowledge of curriculum-based rehabilitative techniques and community resources for clients suffering with severe and persistent mental illness. Physical Requirements: Employee must be able to frequently (up to 85% of the time) sit, stand, walk, and climb stairs (if an elevator is unavailable). Must be able to frequently (up to 90% of the time) talk, listen and speak clearly in person and via telephone. Must be able to push, pull, lift and/or carry up to 10 lbs. in weight on occasion (up to 35%).
