    "description": "<p><span style=\"font-family: arial, helvetica, sans-serif\"><span style=\"font-weight: bold\">Te Korowai Hauora o Hauraki</span> is a not-for-profit kaupapa Māori health organisation, based in Hauraki. Providing medical, wellness and community services in Coromandel, Paeroa, Waihi, Te Aroha and Whitianga areas, we have successfully delivered services in Hauraki since 1994. We aim to be a leading indigenous health provider by providing high-quality, whānau-centred services in the Hauraki locality.</span></p>\n<p><br></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif\">We are looking for a <span style=\"font-weight: bold\">Health Coach</span> to join our <span style=\"color: #222222; font-size: 15px\">Ohu </span><span style=\"color: #222222; font-size: 15px\">Hinengaro (primary mental health) team. This non-clinical role will be based at a designated general practice clinic in Hauraki. </span></span></p>\n<p><br></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif\"><span style=\"color: #222222; font-size: 15px\">Reporting to the Hinengaro Team Lead</span><span style=\"color: #222222; font-size: 15px\">, the role will work</span><span style=\"color: #222222; font-size: 15px\"> closely with our Whānau Health Centre team members, general practice teams, <a href=\"https://www.waikatodhb.health.nz/about-us/a-z-of-services/mental-health-and-addictions/\" target=\"_blank\" rel=\"noopener noreferrer\">Te Whatu Ora - Mental Health &amp; Addiction</a> colleagues and other Te Korowai Hauora o Hauraki services, to ensure whānau needs are met.</span></span></p>\n<p><br></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif\"><span style=\"font-size: 15px\">The Health Coach is actively engaged in utilising an integrated ‘Stepped Model of Care’ that supports whānau needs and enables them to move seamlessly between services. Our Ohu Hinengaro team </span><span style=\"color: #222222\">also uses a </span><a href=\"https://www.mahiaatua.com/\" target=\"_blank\" rel=\"noopener noreferrer\">Mahi a Atua</a><span style=\"color: #222222\"> approach to transforming systems, processes, and outcomes for wh</span><span style=\"color: #222222\">ā</span><span style=\"color: #222222\">nau and is designed to address health inequalities for M</span><span style=\"color: #222222\">ā</span><span style=\"color: #222222\">ori, provide wh</span><span style=\"color: #222222\">ā</span><span style=\"color: #222222\">nau with service choices and improve outcomes for wh</span><span style=\"color: #222222\">ā</span><span style=\"color: #222222\">nau. The model is underpinned by </span><a href=\"https://smiadviser.org/knowledge_post/what-is-feedback-informed-treatment\" target=\"_blank\" rel=\"noopener noreferrer\">Feedback Informed Treatment </a><span style=\"color: #222222\">an evidence-based practice where Mataora/ practitioners gather real-time feedback from wh</span><span style=\"color: #222222\">ā</span><span style=\"color: #222222\">nau using structured measures to identify what is and is not working and then adjust our practice to better meet all wh</span><span style=\"color: #222222\">ā</span><span style=\"color: #222222\">nau needs.</span></span></p>\n<p><br></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif\">We can offer the successful candidate workplace variety, a supportive team and management structure as well as a competitive salary.
