Supports member access to care through face-to-face visits to complete health screening and needs assessments. Certified Community Health Worker Refers member to community resources, processes reports, and distributes collateral materials.</p><p><br></p><p><strong>Competencies/Duties:</strong></p><ul><li>Certified Community Health Worker coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs.</li><li>Will provide telephone and/or on onsite Recovery Support</li><li>Makes referrals to the Case Management Services</li><li>Assesses member compliance with medical treatment plans via telephone or through on-site visits</li><li>Certified Community Health Worker identifies barriers to plan compliance and coordinates resolutions while ensuring alignment with internal company team</li><li>Provide assistance accessing various community resources (education, housing, transportation, recovery supports, etc.)</li><li>Recommends treatment plan modifications and determines need for additional services, in conjunction with case management and provider</li><li>Certified Community Health Worker coordinates identification of and referral to local, state or federally funded programs</li><li>Participates in cross-functional teams on projects, initiatives, and process improvement activities</li><li>Maintains the confidentiality of all sensitive information.
