Under the direct supervision of the CCBHC Clinical Team Lead, the CCBHC Patient Specialist plays a critical role in supporting individuals as they navigate behavioral health, physical health, and social service systems. The CCBHC Patient Specialist will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness.
