Flexible hours, and possibility of full time in the future as we continue to be a first choice in home health &amp; hospice!<br></p>\n<p><br><span>Ideal Candidates:</span><br></p>\n<p>You will love working at First Choice if:</p>\n<ul>\n<li><span>You have a fun and caring personality;</span></li>\n<li>You work well with a team;</li>\n<li>You excel at educating patients and caregivers;</li>\n<li>You are are an effective communicator;</li>\n<li>You put patient care first, and are focused on the best goals and outcomes for the patient</li>\n</ul>\n<p><br></p>\n<p><span>Job Requirements:</span></p>\n<ul>\n<li>LCSW</li>\n</ul>\n<p><br></p>\n<p><span>More Detailed Duties &amp; Responsibilities:</span><br></p>\n<ul>\n<li>Assess patient’s socio-economic, psychological and emotional needs;</li>\n<li>Educate patients and family members regarding resources and act as liaison;</li>\n<li>Provide intervention in crisis situations;<br></li>\n<li>Performs spiritual assessment on all Hospice patients;<br></li>\n<li>Participating in interdisciplinary team meetings;</li>\n<li>Advocating on behalf of the patient and family;<br></li>\n<li>Assists patients and families in preparation of advanced directives;<br></li>\n<li>Assess the need of patient's and families for counseling related to the risk of pathological grief;<br></li>\n<li>Identify patient and family's needs when level of care changes or the patient is discharged<br></li>\n</ul>\n<p><br></p>\n<p><br></p>\n<p><span>Job Type: Part time </span></p>\n<p>Pay: $40-$45/hour</p>",
