Position Description
Northern Pines Mental Health Center is proud to offer competitive wages and an exceptional benefits package!
We are seeking a Behavioral Health Practitioner to join our Assertive Community Treatment (ACT) team. Travel throughout the six-county region will be required. The purpose of this role is to provide elevated, community-based care to adutls with a mental health diagnosis.
Essential Functions include:
Provide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team
Participate in client-centered comprehensive assessment of psychiatric history
Consult with community agencies and families to maintain coordination in the treatment process
Provide on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff
Document client progress to maintain a record of client activity
Participate in daily staff organizational meetings and treatment planning review meetings
Provide ongoing assessment of clients’ mental illness symptoms and clients’ response to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior which put clients at risk (e.g., suicidality)
Provide symptom education to enable clients to identify their mental illness symptoms
Participate in the provision of rehabilitation services, treatment, and supportive interventions
Assist clients to find and maintain a safe and affordable place to live – apartment search, roommate search, landlord negotiations, cleaning, furnishing and decorating, and procuring necessities
Assist and support clients to carry out personal hygiene and grooming tasks
Provide nutrition education and assistance with meal planning, grocery shopping, and food preparation
Ensure clients have adequate financial support (e.g., help to gain employment or apply for entitlements)
Teach money-management skills (e.g., budgeting and bill paying) and assist clients in accessing financial services (e.g., professional financial counseling, emergency loan sources)
Help clients to access reliable transportation (e.g., obtain a driver’s license and car, arrange for cabs, access bus line, find rides)
Assist and support clients to have and effectively use a personal primary care physician, dentist, and other medical specialists as required
Provide side-by-side support and coaching to help clients socialize
Qualifications include:
Travel is a requirement of the position; valid driver’s license and ability to operate a motor vehicle are required
Must meet Mental Health Practitioner status per the State of Minnesota in at least one of the following ways:
Holds a master's or other graduate degree in behavioral sciences or related fields. OR
Has at least 2,000 hours (approximately one year working full-time) of work experience in the delivery of mental health services. OR
Has completed at least 30 semester hours or 45 quarter hours of coursework in Behavioral Sciences or related fields AND has completed a practicum or internship that required direct interaction with adults or children served, and was focused on behavioral sciences or related fields.
Compensation & Benefit s include:
A wage of $25.50/ hour

The purpose of this role is to provide elevated, community-based care to adutls with a mental health diagnosis. \\n\\n \\n\\n Essential Functions include:\\n\\n\\n\\t\\n\\t\\n\\t\\tProvide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team\\n\\t\\tParticipate in client-centered comprehensive assessment of psychiatric history \\n\\t\\tConsult with community agencies and families to maintain coordination in the treatment process\\n\\t\\tProvide on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff\\n\\t\\tDocument client progress to maintain a record of client activity \\n\\t\\tParticipate in daily staff organizational meetings and treatment planning review meetings\\n\\t\\tProvide ongoing assessment of clients’ mental illness symptoms and clients’ response to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior which put clients at risk (e.g., suicidality)\\n\\t\\tProvide symptom education to enable clients to identify their mental illness symptoms\\n\\t\\tParticipate in the provision of rehabilitation services, treatment, and supportive interventions\\n\\t\\tAssist clients to find and maintain a safe and affordable place to live – apartment search, roommate search, landlord negotiations, cleaning, furnishing and decorating, and procuring necessities \\n\\t\\tAssist and support clients to carry out personal hygiene and grooming tasks\\n\\t\\tProvide nutrition education and assistance with meal planning, grocery shopping, and food preparation\\n\\t\\tEnsure clients have adequate financial support (e.g., help to gain employment or apply for entitlements)\\n\\t\\tTeach money-management skills (e.g., budgeting and bill paying) and assist clients in accessing financial services (e.g., professional financial counseling, emergency loan sources)\\n\\t\\tHelp clients to access reliable transportation (e.g., obtain a driver’s license and car, arrange for cabs, access bus line, find rides)\\n\\t\\tAssist and support clients to have and effectively use a personal primary care physician, dentist, and other medical specialists as required\\n\\t\\tProvide side-by-side support and coaching to help clients socialize \\n\\t\\n\\t\\n\\n\\n \\n\\n Qualifications include:\\n\\n\\n\\tTravel is a requirement of the position; valid driver’s license and ability to operate a motor vehicle are required\\n\\tMust meet Mental Health Practitioner status per the State of Minnesota in at least one of the following ways: \\n\\t\\n\\t\\tHolds a master's or other graduate degree in behavioral sciences or related fields. OR\\n\\t\\tHas at least 2,000 hours (approximately one year working full-time) of work experience in the delivery of mental health services. OR\\n\\t\\tHas completed at least 30 semester hours or 45 quarter hours of coursework in Behavioral Sciences or related fields AND has completed a practicum or internship that required direct interaction with adults or children served, and was focused on behavioral sciences or related fields.\\n\\t\\n\\t\\n\\n\\n \\n\\nCompensation & Benefits include:\\n\\n\\n\\tA wage of $25.50/ hour\\n\\tHealth, Dental, Disability and Life insurance with other additional voluntary options for spousal life, short term disability, accident, hospital indemnity and critical illness plans\\n\\tHealth Savings account with employer contribution\\n\\t401K with employer match\\n\\tGenerous paid time off (1st - 3rd year, full-time employees earn 19 days/152 hours per year.
