Additionally, the Case Planner will have the ability to engage youth effectively. The Case Planner will be expected to train and incorporate evidenced based models such as Motivational Interviewing, Conflict Resolution, Mediation and Alia- Intensive Permanency Services (IPS) in delivering services and building relationships with youths as they prepare to transition to independent living. Case Planners will provide services and/or arrange for the provision of social and related services based on service plans, needs and goals as identified through the Readiness Assessment and collaborative feedback;
