If you are ready to be part of an extraordinary team of caregivers, then come work where you matter.</p>\r\n\r\n<p><strong>Hospice Transition Nurse</strong></p>\r\n\r\n<p><strong>Position Overview</strong></p>\r\n\r\n<p>The <strong>Hospice Transition Nurse</strong> is a resource Registered Nurse to patients, family members and staff of hospitals, physicians, and nursing homes. They are responsible for sustaining and increasing referral sources from the hospitals and nursing homes and other community sources. The Hospice Transition Nurse uses their knowledge of St. Croix Hospice programs and services to provide education to these referral sources.</p>\r\n\r\n<p><strong>Essential Functions and Skills</strong></p>\r\n\r\n<ul>\r\n\t<li>Performs as a case manager for patients in the hospital including managing GIP patients when appropriate. Acts as a resource to the hospital and able to navigate hospital EMR systems to locate appropriate documentation needed to support hospice admission.</li>\r\n\t<li>Conducts initial goals of care meetings with patients and families to inform them about St. Croix Hospice programs and services.</li>\r\n\t<li>Collaborates with business development in the community by providing education to facilities and other referral sources regarding Hospice programs and services.</li>\r\n\t<li>Performs follow-up meetings and phone calls from information visits.</li>\r\n\t<li>Sustains and increases referral sources with hospitals, nursing homes and other community sources.</li>\r\n\t<li>Documents visits and phone calls with patients, families, and referral sources.</li>\r\n\t<li>Assists with transition of care for patients out of the hospital that are being transferred home or admitted onto care with St. Croix Hospice including coordination with admission nurse and Hospice Medical Director, coordination of comfort medications and DME needs, and completion of hospice admission consents.</li>\r\n\t<li>Mentors and trains new Registered Nurses as appropriate for high acuity patient needs.</li>\r\n\t<li>Attends Interdisciplinary Care Meetings regarding patients and families in the hospital.</li>\r\n\t<li>Establishes and maintains relationships with hospital Case Mangers, Social Workers, and Physicians including attending any necessary daily rounds.</li>\r\n\t<li>Other duties may be assigned or required for the performance of this position up to and including completing routine visits, admissions and other clinical tasks.</li>\r\n</ul>\r\n",
