The SLP will also be responsible for completing <strong>GG (Functional Abilities and Goals) scoring</strong> as part of the IRF-PAI (Inpatient Rehabilitation Facility &ndash; Patient Assessment Instrument) to track patient progress and meet CMS (Centers for Medicare &amp; Medicaid Services) compliance requirements.</p><p><strong>POSITION&rsquo;S ESSENTIAL RESPONSIBILITIES:&nbsp;</strong></p><p><strong>Clinical Duties:</strong></p><ul><li>Conduct <strong>comprehensive evaluations&nbsp;</strong>for patients with:<ol><li><strong>Dysphagia</strong> (including <strong>clinical swallow evaluations, FEES, or MBSS</strong> if certified)</li><li><strong>Aphasia, dysarthria, apraxia</strong> (stroke, TBI, neurodegenerative diseases)</li><li><strong>Cognitive-communication deficits</strong> (delirium, post-ICU recovery, dementia)</li><li><strong>Tracheostomy patients</strong> (speech &amp; swallow rehab)</li><li><strong>AAC (Augmentative and Alternative Communication) needs</strong></li></ol></li><li>Develop and implement <strong>individualized treatment plans</strong> based on patient needs.</li><li>Collaborate with <strong>physicians, nurses, PT/OT, respiratory therapy, and case management/social worker</strong> for coordinated care.</li><li>Educate <strong>patients, families, and medical staff</strong> on <strong>swallowing precautions, communication strategies, and safe diet recommendations</strong>.</li></ul><p data-pasted=\"true\"><strong>Patient &amp; Caregiver Education:</strong></p><ul><li>Educate patients and families on home safety based on goals of rehab stay.</li><li>Train caregivers and provide guidance on post-discharge exercises and adaptive strategies.</li></ul><p data-pasted=\"true\"><strong>Interdisciplinary Collaboration:</strong></p><ul><li>Participate in <strong>weekly team meetings</strong> to discuss patient progress and discharge planning.</li><li>Communicate effectively with physicians, nurses, case managers, and other therapists.</li><li>Adjust treatment plans based on patient response and medical status.</li></ul><p><strong>GG Scoring &amp; IRF-PAI Compliance:</strong></p><ul><li>Complete&nbsp;GG functional scoring&nbsp;(Section GG of IRF-PAI) for&nbsp;admission, discharge, and periodic intervals.</li><li>Ensure accurate and timely documentation in electronic medical record.</li><li>Participate in&nbsp;interdisciplinary team meetings&nbsp;to review GG scores and adjust care plans.</li><li>Maintain compliance with&nbsp;Medicare, Joint Commission, and facility documentation standards.</li></ul><p><strong>POSITION REQUIREMENTS:&nbsp;</strong></p><ul><li><strong>Education:</strong> Master&rsquo;s or Doctoral degree in Speech-Language Pathology from an accredited program.</li><li><strong>Licensure:</strong> Current state SLP license (or eligible).</li><li><strong>Certification:</strong> Certificate of Clinical Competence (CCC-SLP) from ASHA (or in CFY year).</li><li>Skills:<ol><li>Strong dysphagia management skills&nbsp;(MBSS/FEES preferred but not always required).</li><li>Ability to handle fast-paced, medically complex cases&nbsp;(ICU, stroke, trauma).</li><li>Knowledge of CMS guidelines, IRF compliance, and hospital protocols.</li><li>Proficiency in EMR systems&nbsp;(EPIC, Cerner, Meditech).</li></ol></li><li>Preferred (Not Required but a Plus):<ol><li>LSVT LOUD, MBSImP, or FEES certification.</li><li>Experience with trach/vent patients.</li></ol></li></ul><p><strong>Why Join Legent Health?</strong></p><p>Legent Health fosters an environment where team members are empowered to deliver exceptional care while growing professionally within a supportive, values-driven culture.</p><p><strong>We Offer:</strong></p><ul type=\"disc\"><li>Competitive salary and performance incentives</li><li>Comprehensive benefits package&nbsp;</li><li>Paid time off and wellness programs</li><li>Career development and training opportunities</li></ul><p><strong>Equal Employment Opportunity (EEO) Statement</strong></p><p>Legent Health is an equal opportunity employer.
