This role supports daily clinic operations, coordination of care, and communication with families by assisting providers, managing scheduling needs, and responding to routine clinical questions and concerns. Further, the CNA ensures medical records compliance and complies with agency and departmental policies and procedures. Additionally, the CNA works interdependently with other departments within the agency as well as must be in compliance with the HIPAA and Privacy Health Information Act. QUALIFICATIONS: Education: High School Diploma required. Licensure: Current Ohio CAN required. BLS certification required. CPS training/clearance required. Competency/Skills: Ability to manage parent concerns professionally and demonstrate cultural humility. Core Expertise: Possess skill, knowledge and abilities to perform the essential duties of their role; keeps knowledge up to date. Independent Judgment: Demonstrate ability to perform job responsibilities with a high degree of initiative and independent judgment. Cultural Competency: Demonstrate awareness, sensitivity and skills in working professionally with diverse individuals, groups and communities who represent various cultural and personal background and characteristics. Interpersonal Communication: Communicate clearly using verbal, nonverbal, and written skills in a professional context; demonstrates clear understanding and use of professional language. Professional & Ethical Conduct: Adhere to professional values such as honesty, personal responsibility, and accountability; Applies ethical concepts within scope of work and adheres to Agency policies and procedures. Collaboration and Teamwork: Function effectively as a member of a professional team that includes employees, clients and family members. Problem Solving & Decision Making: Recognize problems and responds appropriately; gathers information and sorts through it to identify and address root cause issues; makes timely decisions. Technical Proficiency: Demonstrate competence in utilizing Agency computer systems and software as required to perform essential job functions Experience: Experience with children, adolescents, and families preferred. Behavioral health or community mental health experience preferred. Scheduling experience in a healthcare setting preferred. Enrollment in an RN program (externship track) preferred. Familiarity with EHR systems preferred. AGENCY SUMMARY: Applewood Centers is a private, nonprofit agency providing mental health services, social services and education to children, adolescents and families living in Cuyahoga and Lorain counties. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment. ESSENTIAL DUTIES: Clinical Support Measure and record vital signs (height, weight, blood pressure, pulse) for patients. Obtain and document basic patient information prior to psychiatric appointments (as directed). Observe and report patient concerns or changes to RN, NP or other provider(s). Scheduling, Patient & Family Communication Schedule new and follow-up psychiatry appointments in coordination with providers and clinic availability. Assist in managing provider schedules, including rescheduling, cancellations, and optimizing clinic flow. Answer incoming calls from parents/guardians and triage concerns using established protocols. Respond to routine clinical questions and provide basic problem-solving for common issues within established guidelines and scope of role. Communicate appointment details and follow-up plans to parents/guardians. Conduct outreach to families for missed appointments, reminders, or rescheduling. Escalate complex, high-risk, or clinically sensitive concerns to RN or provider as appropriate. Coordinate referrals and follow-up services as directed. Contact pharmacies regarding prescriptions, refill requests, and clarifications as explicitly directed by the prescribing provider. Assist with communication between psychiatry providers, therapists, care coordinators, and families. Administrative & Operational Support Assist in preparing psychiatry providers for E&M appointments. Review charts for completeness and flag orders, instructions, or missing information. Scan, upload, and organize documents in the electronic health record. Maintain organization of clinical and administrative workflows. Documentation & Compliance Document interactions and tasks in accordance with agency, legal, and ethical standards. Maintain confidentiality in compliance with HIPAA. Ensure accuracy and completion of required clinical documentation. OTHER DUTIES: Attend scheduled staff meetings, supervision, and on-going training. All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards. Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures. Other duties as assigned by management. Applewood Centers, Inc. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.<br><br><u><strong>ESSENTIAL DUTIES:</strong></u><ol><li>Clinical Support<ul><li>Measure and record vital signs (height, weight, blood pressure, pulse) for patients.</li><li>Obtain and document basic patient information prior to psychiatric appointments (as directed).</li><li>Observe and report patient concerns or changes to RN, NP or other provider(s).</li></ul></li><li>Scheduling, Patient & Family Communication<ul><li>Schedule new and follow-up psychiatry appointments in coordination with providers and clinic availability.</li><li>Assist in managing provider schedules, including rescheduling, cancellations, and optimizing clinic flow.</li><li>Answer incoming calls from parents/guardians and triage concerns using established protocols.</li><li>Respond to routine clinical questions and provide basic problem-solving for common issues within established guidelines and scope of role.</li><li>Communicate appointment details and follow-up plans to parents/guardians.</li><li>Conduct outreach to families for missed appointments, reminders, or rescheduling.</li><li>Escalate complex, high-risk, or clinically sensitive concerns to RN or provider as appropriate.</li><li>Coordinate referrals and follow-up services as directed.</li><li>Contact pharmacies regarding prescriptions, refill requests, and clarifications as explicitly directed by the prescribing provider.</li><li>Assist with communication between psychiatry providers, therapists, care coordinators, and families.</li></ul></li><li>Administrative & Operational Support<ul><li>Assist in preparing psychiatry providers for E&M appointments.</li><li>Review charts for completeness and flag orders, instructions, or missing information.</li><li>Scan, upload, and organize documents in the electronic health record.</li><li>Maintain organization of clinical and administrative workflows.</li></ul></li><li>Documentation & Compliance<ul><li>Document interactions and tasks in accordance with agency, legal, and ethical standards.</li><li>Maintain confidentiality in compliance with HIPAA.</li><li>Ensure accuracy and completion of required clinical documentation.</li></ul></li></ol><u><strong>OTHER DUTIES:</strong></u><ol><li>Attend scheduled staff meetings, supervision, and on-going training.</li><li>All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.</li><li>Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.</li><li>Other duties as assigned by management.</li></ol><p>Applewood Centers, Inc.
