    "description": "<p>Prime Home Health is growing and busy! We are seeking a compassionate and skilled LVN working on PRN basis to join our home health team for in-person, in-home health patient visits in Jourdanton, Pleasanton and surrounding areas. As an LVN on our team,  you will play a vital role in providing comprehensive nursing care to adult home health patients.  You will work closely with the interdisciplinary team to ensure the highest quality of care.  <span style=\"font-weight: bold;\">This is not a remote position, you must live in the Jourdanton, Pleasanton or surrounding area in order to see patients in person. </span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold;\">Responsibilities:</span></p>\n<p><br></p>\n<ol>\n<li>Patient Care: Provide direct patient care in accordance with the established plan of care and individualized patient needs, including administering medications, performing treatments, monitoring vital signs, and managing symptoms. <br></li>\n<li>Assessments and Documentation: Conduct thorough assessments of patients' condition, document observations, and maintain accurate and up-to-date medical records, including progress notes, medication administration records, and treatment plans.</li>\n<li>Education and Support: Educate patients and their families about their condition, treatment options, and end-of-life care, providing emotional support and guidance to help them cope with the challenges they may face.<br></li>\n<li>Collaborative Communication: Maintain open and effective communication with the interdisciplinary team, patients, families, and caregivers, ensuring the coordination of care, addressing concerns. <br></li>\n<li>Safety and Compliance: Adhere to infection control protocols, maintain a safe environment for patients and staff, and comply with all relevant laws, regulations, and organizational policies and procedures.</li>\n<li>Professional Development: Stay updated with the latest developments in hospice care, attend educational programs, and participate in ongoing professional development activities to enhance knowledge and skills.</li>\n</ol>\n<p><span style=\"font-weight: bold;\">Qualifications:</span></p>\n<ol>\n<li>Valid LVN license issued by the State of Texas.</li>\n<li>You must live in the Jourdanton, Pleasanton, Poteet or surrounding areas. </li>\n<li>Minimum of one year of experience as an LVN providing in-home health patient visits.</li>\n<li>Experience using Kinnser/WellSky EMR, (or others), efficient with apps such as Slack and others. </li>\n<li>Current CPR BLS certification</li>\n<li>Strong clinical skills and knowledge of nursing principles and practices.</li>\n<li>Excellent communication and interpersonal skills to interact effectively with patients, families, and the interdisciplinary team.</li>\n<li>Professional, compassionate and sensitivite to the physical and emotional needs of our patients and their families.</li>\n<li>Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.<br></li>\n<li>Valid driver's license and reliable transportation for home visits.</li>\n<li>Commitment to maintaining confidentiality and upholding ethical standards in patient care.</li>\n</ol>\n<p><br></p>\n<p>We offer a professional, supportive administrative team and competitive pay and mileage or trip fees for our PRN staff and have several immediate openings in all areas of Jourdanton and San Antonio.
