Description

In Home Co-Therapist Job Description: The In-Home Co-Therapist for the FIND program works with adolescents and their families. The In-Home Co-Therapist will work with a Lead Therapist to provide individual, parenting, and family therapy sessions to adolescents who are on probation and their families. The In-Home Co-Therapist will also provide crisis intervention to the individual and families when needed. What you'll be doing: Assist Therapist in assessing & implementing treatment program for clients and their families Review referral source information to determine needs/priorities in therapy or in case mgmt. Provide client’s family immediate intervention in situations where a client faces out of home placement Facilitate the reunification & reinforce client back into his/her family or other less restrictive environment when out of home placement has occurred Assist in providing and presenting assessments, treatment goals and progress towards goals at staff meetings Collaborate closely with the Robert Neaves Center, internal staff at Nova and other community agencies that work with the designated client Provide accurate and timely documentation in clients electronic records on a weekly basis to include case formulations, intakes, progress notes, etc. Perform crisis intervention as necessary Adhere to and fulfill requirements as outlined by the DMH/MR State Standards of Care, Federal and State Regulations as well as MHC Policies Work with Outreach Program Manager and co-therapist to assist family on a 24/7 call basis Complete other duties as assigned.
