This role coordinates all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling of residents and patients in a variety of settings.</span></span><span style=\"font-size: 12px; font-family: georgia, serif; color: rgb(0, 0, 0);\"><br></span></p><ol><li style=\"font-family: georgia, serif; font-size: 12px;\">Ability to work effectively in a collaborative environment.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Ability to communicate effectively with patients, residents, and families of diverse backgrounds who may have physical, sensory and mental impairments.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Complies with all state and federal safety, health and sanitary regulations as well as St. Camillus policies and procedures.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Collaborate with providers, residents and patients to develop nutritional plans of care.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Complete initial nutritional evaluations of residents and patients and assesses and monitors nutritional risk.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Reviews and revises nutritional care plans and schedules as needed.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Coordinates changes in nutritional care planning with staff, MDs, nurses, food service, and make regular follow-ups.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Makes recommendations to providers regarding orders, monitoring and tolerance for enteral and parenteral nutrition. Completes follow up assessments monthly or when indicated.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Educates residents, patients and families regarding prescribed diet and nutrition plan of care. Provides written materials to families, residents/patients, and/or caregivers, when appropriate. Assess compliance and understanding of diet restrictions.</li><li style=\"font-family: georgia, serif; font-size: 12px;\">Assists in menu planning and recipe analysis.
