Intake Coordinator Contract up to 18 months, Full-time (35 hours per week) Internal/External Posting The Alzheimer Society of Peterborough, Kawartha Lakes, Northumberland and Haliburton is seeking a compassionate and detail-oriented professional to join our team as an Intake Coordinator. In this role, you will be the first point of connection for individuals and families seeking support. You will play a key role in creating a welcoming, respectful, and person-centred experience, helping people feel heard, informed, and supported from their very first interaction with our organization. About the Role As the Intake Coordinator, you will receive referrals and connect with care partners, and individuals living with dementia to gather important information to help guide their journey with our services. You will complete components of the First Link Standardized Assessment and support data collection to advance health equity. You will work closely with the Counselling and Navigation team to ensure individuals are connected to the right supports at the right time. Key Responsibilities Respond to email, phone, and in-person inquiries for support and services Provide information about Alzheimer Society programs and services Receive and process professional and self-referrals Complete portions of the First Link Standardized Assessment Gather information about individuals’ circumstances and support needs in a person-centred, non-judgemental manner Collect information to support health equity initiatives Review privacy, confidentiality, and consent for service with new clients Coordinate completion of consent and communication documentation Provide welcome/information packages and follow up within established timelines Accurately document client interactions and information in the client database Collaborate with the Counselling and Navigation team to ensure appropriate service connections Occasionally provide coverage for social recreation programs, as needed Who You Are You are a confident, compassionate communicator who excels at building rapport quickly. You bring a calm, organized approach to your work and are comfortable managing multiple priorities in a fast-paced environment. You’ll thrive in this role if you: Enjoy being the first point of contact and creating positive first impressions Are detail-oriented and comfortable managing administrative processes Value a person-centred approach and treat every interaction with empathy and respect Work well both independently and as part of a collaborative team Qualifications Diploma or degree in Social Work, Social Services, Geriatric Care, or a related field (or equivalent experience) Preference given to regulated professionals in good standing with an Ontario regulatory body Experience Supporting individuals living with dementia and their care partners Assessment, care planning, or service coordination Working in healthcare, human services, or inter-professional environments Skills & Competencies Knowledge of dementia care and person-centred practices Excellent interpersonal and communication skills Strong organizational and time-management abilities Cultural sensitivity and awareness of diversity Knowledge of community, health, and social service systems in the region Proficiency in Microsoft Office and client databases Additional Requirements Valid G Class driver’s license and access to a reliable, insured vehicle Vulnerable Sector Police Check (within the last 2 months) Membership in a relevant regulatory body is an asset Working Conditions This role primarily takes place in office and community settings, with travel throughout Peterborough, Kawartha Lakes, Northumberland, and Haliburton. Occasional evening and weekend work may be required. Our Compensation Package Grade 4 position with starting salary commensurate to experience: $22.00 - $25.00 per hour (35 hours per week) Access to the Employee Assistance Program ( *excludes LTD ) 15 days annually (prorated for contract roles), accrued monthly, in addition to office closure between December 25 and January 1. 5 paid and 5 additional unpaid emergency leave days per year In the summer months, flex time may be used to take Friday afternoons off making the most of the great weather and everything our four counties have to offer.
