Social Worker - Wellness Together
University Settlement is one of New York City's most dynamic social justice institutions, with an impressive legacy as the first settlement house in the nation. For over 135 years, University Settlement has been an anchor in the low-income and immigrant communities where we work, offering pioneering programs in early childhood care and education, youth development, eviction prevention, literacy, theater and visual arts, older adults services, and mental health. Each year, we engage over 40,000 New Yorkers through our network of 30 sites in Manhattan and Brooklyn. Our mission is to empower residents by building on their strengths through comprehensive, quality services that meet the current needs of the community; innovation that anticipates future needs; and advocacy to bring justice to the community as a whole.
Wellness Together aims to improve the quality of life for older adults who are homebound due to age or infirmity. Through comprehensive case management, various virtual group activities, and volunteer services, the program supports older adults to live their lives to their fullest potential. The program is part of the agency’s LEARN (Longevity through Education, Arts, Recreation, and Nutrition) network that works with older adults.
The Social Workers will spend the majority of their time in the community through participant home visits and partnership collaborations.
Responsibilities:
Provide intake, on-going assessment, and develop individualized, goal-oriented service plan to ensure participants receive the services they need to ensure good health and wellness in their homes
Screen and assess clients for social determinants of health, mental health, trauma, risk, and substance use
Provide crisis intervention, prevention, and resolution/
Perform outreach, information, case assistance, entitlement evaluation, referrals, and advocacy to link services to homebound older adults to promote successful community living
Maintain relationships with partner organizations and assist with collateral services delivery and follow-up
Conduct outputs and outcome evaluation as required by funders and program; assist in program planning and evaluation
Assist with virtual programming activities designed for homebound older adults
Complete data entry into appropriate database systems and CRM platforms daily, weekly, and monthly.
Keep client files up to date and provide regular statistical reporting on outcomes .
Give feedback on newly designed documents or forms for record keeping and outreach purposes
Coordinate with staff for quality control, integration of services, and program outcomes measurements
Opportunity to provide weekly supervision for social work interns as needed
Facilitate peer supervision sessions
Perform other tasks consistent with this job description as assigned by supervisor
Qualifications:
Master of Social Work or related field required
Ability to build and maintain professional helping relationships required
Ability to relate and communicate with diverse population and groups required
Resilience and ability to assess unexpected, real time, situations required
Excellent skills in Microsoft Office
Must have strong attention to detail and able to work independently
Must demonstrate strong commitment to working in a multi-cultural environment
Excellent communication skills
Prior experience working with older adults preferred
Bilingual language skills required, must be fluent in written and spoken in one or more of the following languages (Spanish, Cantonese, Mandarin, Polish, Ukrainian, Russian, Italian, language requirements will change based on needs.

The program is part of the agency’s LEARN (Longevity through Education, Arts, Recreation, and Nutrition) network that works with older adults. \\n\\n The Social Workers will spend the majority of their time in the community through participant home visits and partnership collaborations.\\n\\n \\n\\nResponsibilities: \\n\\n\\n\\tProvide intake, on-going assessment, and develop individualized, goal-oriented service plan to ensure participants receive the services they need to ensure good health and wellness in their homes\\n\\tScreen and assess clients for social determinants of health, mental health, trauma, risk, and substance use\\n\\tProvide crisis intervention, prevention, and resolution/\\n\\tPerform outreach, information, case assistance, entitlement evaluation, referrals, and advocacy to link services to homebound older adults to promote successful community living\\n\\tMaintain relationships with partner organizations and assist with collateral services delivery and follow-up\\n\\tConduct outputs and outcome evaluation as required by funders and program; assist in program planning and evaluation\\n\\tAssist with virtual programming activities designed for homebound older adults\\n\\tComplete data entry into appropriate database systems and CRM platforms daily, weekly, and monthly.\\n\\tKeep client files up to date and provide regular statistical reporting on outcomes.\\n\\tGive feedback on newly designed documents or forms for record keeping and outreach purposes\\n\\tCoordinate with staff for quality control, integration of services, and program outcomes measurements\\n\\tOpportunity to provide weekly supervision for social work interns as needed\\n\\tFacilitate peer supervision sessions\\n\\tPerform other tasks consistent with this job description as assigned by supervisor\\n\\n\\n \\n\\nQualifications: \\n\\n\\n\\tMaster of Social Work or related field required\\n\\tAbility to build and maintain professional helping relationships required\\n\\tAbility to relate and communicate with diverse population and groups required\\n\\tResilience and ability to assess unexpected, real time, situations required\\n\\tExcellent skills in Microsoft Office\\n\\tMust have strong attention to detail and able to work independently\\n\\tMust demonstrate strong commitment to working in a multi-cultural environment\\n\\tExcellent communication skills\\n\\tPrior experience working with older adults preferred\\n\\tBilingual language skills required, must be fluent in written and spoken in one or more of the following languages (Spanish, Cantonese, Mandarin, Polish, Ukrainian, Russian, Italian, language requirements will change based on needs. \\n\\n\\n \\n\\nWe are committed to building a diverse and inclusive community.
