Description Aviva Family and Children’s Services Mission Statement Creating safe, supportive spaces, where children, youth and families can heal, build resilience, and experience belonging. Job Summary The High-Fidelity Wraparound (HFW) Therapist provides intensive, individualized, and family-centered mental health services following the HFW model, emphasizing family voice and choice, team-based planning, and culturally responsive care.  This role focuses on supporting children and youth with complex behavioral, emotional, and mental health needs by coordinating care, facilitating team-based planning, and delivering therapeutic interventions within the home, school, and community settings.  The HFW therapist is part of a multidisciplinary team that works collaboratively with caregivers, natural supports, and system partners to develop individualized care plans that promote stability, permanency, and overall well-being.  The HFW therapist is available 24/7 for crisis response.  Additionally, the HFW Therapist completes all necessary documentation in accordance with Aviva, DMH, DCFS and COA standards. Essential Functions Conduct initial and ongoing assessments of child/youth and family needs and strengths, utilizing the CANS for prioritization and progress tracking Identify underlying needs and trauma as the driving force of service delivery, ensuring they are reflected in the individualized service plan Develop, maintain, and adapt treatment plans collaboratively with the family, natural supports, and service providers Monitor client progress and adjust interventions based on outcomes and data Provide individual, family, and group therapy in home, school, and community settings Implement trauma-informed clinical interventions tailored to each child/youth's underlying needs  and educate parents/caregivers Actively participate in, and as needed coordinate and facilitate, Child and Family Team (CFT) meetings, contributing a clinical lens to team planning Actively participate/collaborate in team meetings and case conferences for optimal mental health prognosis of clients Provide comprehensive treatment services which may include interfacing with community psychiatric hospitals, Department of Family and Children Services (DCFS), Department of Mental Health (DMH), school officials, probation department, court system, law enforcement and others as appropriate. Provide psychoeducation to caregivers on managing behavior and responding to trauma responses while preserving therapeutic confidentiality Equip families with tools to support the child/youth's strengths and positive contributions Implement the HFW process (engagement, planning, implementation, transition) Collaborate with treatment team, family, natural supports, and other system/service providers to develop treatment plans that are strength-based and culturally responsive Complete accurate and comprehensive assessments, progress notes, and treatment documentation within required timelines Complete ongoing evaluation of needs by utilizing applicable outcome measures LPS designation by Los Angeles County Department of Mental Health is required for this position and includes all associated responsibilities including but not limited to assessment, writing holds when necessary, coordinating services for hospitalization and transportation Deliver crisis intervention and stabilization services, including creating and regularly reviewing Crisis Safety Plans Communicate crisis situations, e.g. psychiatric hospitalizations, Suspected Child Abuse, etc. to team and supervisor as soon as possible. On-call 24 hours per day, 7 days a week to provide crisis support, by phone and/or in-person if indicated Assess the need(s) to request flex funds and communicate the need(s) to treatment team, address the need(s) in the CFT meeting, and (engagement, teaming, assessment and understanding, long term view, planning, cultural competence, transitions, crisis management). Support families in building natural supports and community connections Monitor client progress and adjust interventions and treatment plans based on outcomes and data to ensure positive outcomes Maintain service time expectations and accurate and timely documentation, reports and outcome measures in compliance with agency and regulatory standards Consult and serve as a liaison with community agencies, school personnel, parents and other mental health programs and services to evaluate community health needs and services Educate others on needs of specific client or segment client’s mental health population Advocate for client needs within systems and across service providers Apply trauma-informed, culturally responsive practices across all client interactions Build protective capacity with families using Core Practice Indicators Attend regularly scheduled staff meetings, in-service trainings and other meetings as appropriate. Maintain client confidentiality in compliance with HIPAA and all applicable regulations Participate in staff meetings, in-service trainings, and quality improvement projects The HFW Therapist will take on special projects or additional duties as assigned by the ICBS Leadership. Qualifications Competency Requirements Ability to fluently speak, read, write, and understand English Bilingual in Spanish a plus Effective oral and written communication skills, including thorough clinical documentation and medical record-keeping. Knowledge of community mental health treatment principles, including interviewing, assessment, and individual, family, and group counseling. Knowledge of general psychiatric emergency intervention, diagnosis, and brief and long-term therapy methods. Knowledge of adult and child psychology. Strength-based, family-driven, and youth-guided approach to care. Ability to assess crisis situations and respond in a timely and professional manner, including de-escalation techniques. Ability to work effectively with families of diverse ethnic and cultural backgrounds. Cultural humility, responsiveness, and trauma-informed care. Knowledge of laws and regulations related to patient rights, voluntary/involuntary treatment, and commitment criteria. Knowledge of Medi-Cal regulations and documentation for medical necessity. Knowledge of community resources and agencies providing support services for individuals who are mentally ill or disabled. Ability to work effectively as a member of a multidisciplinary treatment team, including team facilitation, leadership, and collaboration. Strong organizational, planning, time management, and problem-solving skills. Education and Experience Master’s degree in Human Services, Social Work, Psychology, or related field required Must be registered with the Board of Behavioral Sciences Experience conducting individual therapy with high-risk youth and their families required. Experience with crisis management and intervention highly preferred. Knowledge of the HFW model, Intensive Care Coordination (ICC), and Intensive Home-Based Services (IHBS) and systems of care preferred. Strong ability to engage diverse families, collaborate across systems, and manage crises effectively. Ability to work flexible hours, including evenings and weekends. Valid driver's license and reliable transportation required. Certifications/Licenses Valid CA driver’s license Valid intern number or license (ASW, MFTI, LCSW, LMFT) with Board of Behavioral Sciences Additional Requirements Tuberculosis clearance Department of Justice/Fingerprint clearance Child Abuse Index clearance Satisfactory completion of health screening Proof of legal eligibility to work in the United States Satisfactory driving record Delegates Authority To N/A Physical Demands While performing the duties of a Wraparound Therapist, the employee is frequently required to walk, drive, sit, and use their fingers to manipulate a cell phone, keyboard or writing instrument.
