    "requisitionDescription": "<div><div><div><div><div><div><div><div><div><p><span style=\"font-family: arial, sans-serif; font-size: 16px;\">David Raines Community Health Centers is looking for passionate, qualified applicants to join our team!</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 16px;\">The<strong>&nbsp;PRN Licensed Professional Counselor</strong> provides counseling services on an &quot;as needed&quot; basis. The PRN LPC is responsible for coordinating Social Service planning and other services that assist the patient and their family in understanding and coping with emotional and social problems that affect their health status. The PRN LPC will provide intake and patient advocacy services, as well as coordinate social service matters with physicians, staff, patient, families, and other organizations.</span></p><p><span style=\"font-family: arial, sans-serif; font-size: 16px;\"><u><strong>Essential Skills</strong></u></span></p><ul id=\"isPasted\"><li style=\"font-family: arial, sans-serif; font-size: 16px;\">Identifies and addresses psychosocial needs to students through assessment, consultation, counseling, and case management.</li><li style=\"font-family: arial, sans-serif; font-size: 16px;\">Collaborates with multidisciplinary team to promote a holistic approach to healthcare.</li><li style=\"font-family: arial, sans-serif; font-size: 16px;\">Collaborates with community services to provide continuity of care.</li><li style=\"font-family: arial, sans-serif; font-size: 16px;\">Provides mental health education for students, families, teachers, professional peers, and the community.</li><li style=\"font-family: arial, sans-serif; font-size: 16px;\">Performs case management duties and maintains documentation that meets standards for health system, Office of Public Health, Board of Social Work, and other supervising entities.</li><li style=\"font-family: arial, sans-serif; font-size: 16px;\">Contributes to performance improvement initiatives (development, data collection, LaPERT, etc.).
