This temporary program combines structured clinical assessment, individualized support planning, and daily therapeutic services in a highly supportive and structured environment.</p>\n<p>Youth typically remain in care for <span style=\"font-weight: bold\">about 30 days</span>, during which they receive intake, multidisciplinary evaluation, education, independent living and social skills training, and initial mental health stabilization. You will coordinate treatment activities, facilitate communication between stakeholders, and help youth and families prepare for successful reintegration into their communities.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Key Responsibilities</span><br>• Provide case management services, including communication with youth, families, and Juvenile Probation Officers<br>• Complete or coordinate required mental health and substance abuse assessments<br>• Develop and update treatment plans, aftercare plans, performance summaries, and discharge summaries<br>• Support data collection for post-release tracking and outcomes<br>• Serve as liaison between the program, DJJ, families, and external agencies<br>• Prepare families for visitation, home leave, aftercare, and group/family meetings<br>• Connect youth and families to appropriate community resources<br>• Participate in internal audits and reviews to ensure policy compliance</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Qualifications</span><br><span style=\"font-weight: bold\">Required:</span><br>• Bachelor’s degree in counseling, social work, psychology, rehabilitation, or related human services field<br>• Valid driver’s license and ability to pass background checks<br>• Emotionally and physically able to engage in youth activities and program duties</p>\n<p><span style=\"font-weight: bold\">Preferred:</span><br>• Two years of experience working with youth with emotional or substance use challenges</p>\n<p><span>•  If the candidate has a master’s degree in an appropriate field, the experience requirement is preferred, but not            required.</span><br>• Experience in juvenile justice, Medicaid-funded environments, or group facilitation<br>• Familiarity with treatment planning, case documentation, and family engagement strategies</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Apply Today</span><br>Make a difference with us.
