As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. Trinity is accredited by DNV Healthcare Inc. and has achieved Pathway to Excellence designation by the American Nurses Credentialing Center. </p>\n<p><strong><em> </em></strong></p>\n<p><strong><u>General </u></strong></p>\n<ul>\n <li>Flexible role (full time and part-time options available)</li>\n <li>Established team of 1 Medical Director and 4 Advanced Practitioners  </li>\n <li>Established practice serving the community</li>\n <li>Experienced staff, both clinical and non-clinical, who know the patients and the community</li>\n <li>Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination</li>\n</ul>\n<h2>Responsibilities</h2>\n<ul>\n <li>Full & part-time roles</li>\n <li>Clinic hours: Monday-Friday 8am-5pm\n  <ul>\n   <li>4 day work week </li>\n   <li>No nights, weekends, or holidays</li>\n  </ul></li>\n <li>Palliative Care services provided in patient's homes, clinic and/or nursing home setting </li>\n <li>Average of 8-10 patients per day; no nights or weekends </li>\n <li>Shared call (telephone only) amongst the providers.
