If you are a Healthcare profesional who wants to work in a meaningful, mission-driven workplace, Optimus may be for you!&nbsp;</p><p>This role includes one evening until 7:30 per week and&nbsp; participates in a telephonic call and Saturday rotations ( 9-1:00).&nbsp;</p><p id=\"isPasted\">&bull; &nbsp; &nbsp;Multispecialty Integrated Care Practice</p><p>&bull; &nbsp; &nbsp;100% Outpatient Setting</p><p>&bull; &nbsp; &nbsp;Excellent Fringe Benefit &amp; Health Benefits</p><p><strong>&bull; &nbsp; &nbsp;Competitive salary &amp; sign-on/retention bonus up to $25,000</strong></p><p>&bull; &nbsp; &nbsp;Federal Student Loan Repayment, if eligible</p><p id=\"isPasted\">The Integrated Care Nurse Practitioner / Advanced Practice Registered Nurse is delegated the authority by their supervising physician to perform physical examinations and order diagnostic testing including but not limited to laboratory tests, diagnostic imaging tests; to make clinical assessments, and implement a plan of treatment, to include, but not limited to, the prescribing of medication as so granted by Connecticut statutes. &nbsp;It is understood that the authority to perform these delegated tasks outlined above comes from the licensed supervising physicians within Optimus who shall provide oversight over the clinical activities of those individuals under their authority. &nbsp;The Integrated Care Nurse Practitioner / Advanced Practice Registered Nurse may serve as primary care provider within certain departments, and as such, may carry their own patient panels.&nbsp;</p><p>FUNCTIONS &amp; RESPONSIBILITIES</p><p>1.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Obtain complete medical history and physical data on patients.</p><p>2.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures as needed.</p><p>3.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Perform complete physical exam and records findings in electronic medical record. Collaborates with physicians as needed in managing acute and long-term medical needs of the patients.</p><p>4.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Order appropriate laboratory and diagnostic procedures.</p><p>5.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Synthesize data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.</p><p>6.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Administer medications and injections as needed.</p><p>7.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Interview and advise patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.</p><p>8.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Instruct patient and family regarding medications and treatment instructions. Provides patient education.</p><p>9.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Maintain and review patients&rsquo; records, charts, and other pertinent information.
