  "BriefDescription": "The Case Manager is responsible for the implementation of the strategies from the Person Centered Plan and Individual Service Plans for each individual client assigned to them.  This includes obtaining and maintaining all supporting documents for the PCP and ISP, the continual assessment of each client's needs and choices, monthly review and revision of the PCP and ISP and providing regular documentation and feedback to regulatory agencies, residential providers, and guardians.  The Case Manager is responsible for the provision and oversight of training and community services as well as admissions and also serves as liaison for OIG.  They will work with the Director and Assistant Director to insure appropriate resources and materials are available for implementation of activities.",
