Title: Case Manager II, Substance Use Disorder
Department: Behavioral Health
Reports To: Director, Harm Reduction Program
Location: 56 Bay St., 3rd Floor
Position Status: Full Time
FLSA Status: Non-Exempt
Pay Rate/Benefits : The pay range for this role is generally $41,600 - $46,600 annually , commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI ) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.
POSITION SUMMARY:
The Hotspotting Opioid Program is an evidence-based program in which a care management model will be led by certified recovery peer advocates and clinical professionals with the objective of providing person-centered services to those at risk of opioid overdose in a safe and collaborative way. The program’s focus is to reduce opioid harm, promote a continuum of recovery, and address critical social determinant of health factors.
Under the supervision of the Director of Harm Reduction, the individual will be responsible for providing client outreach, interventions for clients, provide crisis consultations, maintain client case records, linkages to care, referral follow up. Case Managers will document interventions according to all funder and agency requirements, submit statistical data and reports, attend weekly supervision, participate and facilitate case conferences, conduct staff trainings on substance abuse topics, and perform other additional tasks as delegated by supervisor.
DUTIES & RESPONSIBILITIES:
Perform outreach to potential clients to engage them into services. Coordinate the implementation of the multidisciplinary treatment plan, with treatment team. Document all client activities, progress, and clinical intervention in the case record. Coordinate treatment services with internal programs and external organizations. Address and remove barriers to enable client access to all necessary components of health care, including mental health and substance use services by coordinating with the treatment team. Maintain ongoing face-to-face, telephone, and other collateral contacts to support, encourage, and promote the client's commencement and ongoing compliance with the requirements of treatment and/or services outlined by CHASI and the funder, as well as to assist with mitigating barriers to engagement in services, wellness, and recovery, as well as employability by working closely with the treatment team. Use required systems to track outreach and engagement activity; conduct Health Related Social Needs Assessments (HRSN) assessments using required platforms and tools and make referrals to treatment and recovery services Assist clients in coordinating a manageable treatment. Help to collect documentation for SSI and housing applications as well as provide advocacy services throughout the application process. Coordinate logistics for care and treatment needs including appointment reminders and consistent follow-up. Assess plan and treatment adherence and addresses barriers as they arise. Connect clients to Harm Reduction services including but not limited to naloxone training and kits, and fentanyl test strips. Reassess client needs and eligibility for the program. Develop and discuss aftercare plans with clients, referral resources, and other relevant entities. Performing crisis intervention and problem resolution for clients in need. Coordinating efforts with treatment staff and documenting same in the case record. Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values. Other duties as assigned.
