JOB DESCRIPTION
Summary/ Objective
East Side House (ESH) is a community resource in the Bronx and Northern Manhattan. We believe education is the key that enables all people to create economic and civic opportunities for themselves, their families, and their community. ESH’s Adult Programs align with our mission by providing high quality programs and services within community centers and school partnerships.
Reporting to the Program Director, t he Social Worker is integral to East Side House’s strategy to organize resources and share leadership to integrate social services into the fabric of the programs and communities we serve. Social Workers serve as the primary leaders of wrap-around services for program participants and families, design and implement (proactive and preventive) social work programs to promote social and emotional health, respond to day-to-day participant issues and unanticipated crises and their most immediate needs. This position’s direct reports include two Home Visitors and interact with participants from the Home-based Expectant Families and Early Head Start programs. The Social Worker provides assessments, treatment planning/referrals, emergency and crisis intervention and overall coordination of care – improving mental well-being, strengthening relationships and promoting life skills to assist participants with overcoming barriers preventing goal attainment.
Essential Functions and Responsibilities
Under the supervision of the Program Director, t he Social Worker, will be responsible for but not limited to the following:
Complete initial needs assessment with participant/family.
Build relationships with participating clients through consistent individual/group counseling sessions or mediations.
Develop plans for interventions and will include, but not limited to, gathering information from program leaders, staff, and members.
Use validated tools to screen consenting members for mental health symptoms and re-screen members at appropriate intervals (which will vary, based on population needs and the interventions selected) to identify changes in symptoms and adjust clinical services.
For psychoeducation, facilitate one-on-one sessions and/or group sessions, based on the evidence-based intervention selected.
Provide on-site clinical counseling services for participants. On-site services would be delivered in a manner that ensures privacy and confidentiality for those receiving services.
Provide community referrals for participants and families as needed. Follow up on referrals to determine outcomes and help resolve barriers. Provide linkage between participant/family and community resources.
Provide clinical counseling and connections to additional treatment as needed.
Make recommendations related to methods to promote engagement to increase program participation.
Ability to maintain a HIPAA compliant professional work environment.
Develops and maintains positive relationships with residents and their families and aids when needed.
Completes appropriate paperwork and recommends improvements and more efficient ways of operating the department.
Serve as an internal liaison to link participants and families offered throughout ESH’s programs (i.e., childcare, after school, community and school-based programs, workforce, post-secondary, etc.)

Qualifications
Qualifications
To perform this position successfully, the Social Worker must be able to fulfill all the elements of the job description to the highest standard and be able to work well with colleagues in a high energy and collaborative work environment. Additional qualifications include:
Commitment to demonstrating ESH’s organizational values of Excellence, Integrity, Compassion, Empathy, and Community
SIFI Certification strongly preferred with at least one year’s experience supervising social work interns
Knowledge of the network of social services available to parents and families in New York
Bilingual in English and Spanish
Education / Experience
Licensed Clinical Social Worker (LCSW); Licensed Master Social Worker (LMSW); or Licensed Mental Health Counselor (LMHC); and
At least three (3) years of successful and relevant supervisory experience and direct experience in mental health counseling, e.g. trauma-informed care
Competencies
Experience working with community served by East Side House Settlement
Strong organizational skills and ability to coordinate multiple tasks and responsibilities
Capacity to work independently and collaboratively with staff members
Diverse cultural competence required
Interest in and comfort with working with data informed practice
Computer proficiency including Microsoft Word and Excel, email correspondence, and database management a requisite
Clearance
Employees must be clear to work at ESH. Employment is contingent upon successful completion of background checks and/or screenings.
Supervisory Responsibility
This position is responsible for supervising and managing the performance of all designated employees assigned to the position within the department.
Physical Demands
This role may involve occasional lifting or moving of items up to 25 pounds and routine use of standard office equipment. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.
Expected Work Schedule
The work schedule expected for this position is: 8:00am – 4:00pm Monday - Friday, as aligned to the needs of the program/service. The s chedule may be adjusted based on ESH’s operational needs and funder requirements, with notice provided whenever possible.

This position’s direct reports include two Home Visitors and interact with participants from the Home-based Expectant Families and Early Head Start programs.
