Description
Job purpose
The purpose of the Case Manager position is to support children and/or adults with Serious Mental Illness (SMI) in achieving stability, safety, and improved functioning by coordinating services, monitoring progress, and advocating for needed supports. The Case Manager ensures timely engagement, consistent service delivery, and accurate documentation in compliance with agency standards, state regulations, and billing requirements. Through client-centered planning, collaboration with families and providers, and connection to community resources, the Case Manager promotes continuity of care and helps clients progress toward their individual goals.
Duties and responsibilities
Engage clients and families in ongoing, client-centered case management services that support stability, safety, and progress toward identified goals.
Schedule all client appointments monthly in Empathic as billable services to ensure required contact and billing standards are met.
Contact new clients within 1–2 business days to complete intake and schedule the initial appointment.
Monitor attendance and make timely efforts to reschedule missed or canceled appointments within the same month.
Complete, maintain , and update required assessments and service plans, including Functional Assessments, Crisis Plans, Individual Family and Community Support Plans (IFCSP), CASII assessments, and Diagnostic Assessments, within required timelines.
Document all client, collateral, and provider contacts accurately and within three business days, including attempted contacts and missed appointments.
Coordinate and monitor services across the local system of care, including mental health providers, schools, social services, and community resources.
Participate in regular supervision to review service delivery, documentation, and compliance with agency and state requirements.
Submit Requests for Service Authorizations and required documentation at least 30 days prior to expiration .
Follow agency protocols for client discharge, transitions, and case closures.
Maintain a caseload of approximately 12 –15 clients and provide coverage for other Case Managers as assigned.
Refer clients to crisis services when emergencies occur outside of business hours, in accordance with agency policy.
Attend required trainings and complete supervision, mentoring, and continuing education requirements.
Track and submit mileage and parking expenses within required timelines.
Adhere to ethical standards, confidentiality requirements, and agency policies.
Case Managers do not transport clients or family members in personal vehicles.
Qualifications
Candidates must meet one or more of the following criteria in accordance with Minnesota statute:
Experience and training in working with children with mental health needs.
Qualify as a Mental Health Practitioner under Minn. Stat. §245I.04, subd. 4; OR
Hold a bachelor’s degree in a behavioral science or related field (including, but not limited to, social work, psychology, nursing) from an accredited college or university; OR
Meet one of the alternative qualification pathways outlined below.
Applicants who are not mental health practitioners and do not hold a qualifying bachelor’s degree may qualify if they meet one of the following:
Three or four years of experience as a Case Manager Associate (CMA) ;
Registered Nurse without a bachelor’s degree with at least three years of combined psychiatric training and relevant community-based mental health experience;
Previously qualified as a case manager under the 1998 Department of Human Services waiver and meet ongoing supervision and training requirements;
Completion of at least 80 hours of specialized training on the characteristics and needs of children with Serious Mental Illness (SMI) prior to providing direct services;
Demonstrated competency in practice and knowledge of children’s Serious Mental Illness consistent with national practice standards prior to service delivery.
Supervision, Training + Continuing Education Requirements:
Case Managers with 2,000 or more supervised hours delivering mental health services to children must receive 38 hours of supervision annually , including at least one hour per month of clinical supervision .
Case Managers without 2,000 supervised hours must:
Complete 40 hours of DHS-approved training prior to providing services; and
Receive weekly clinical supervision from a mental health professional until the experience requirement is met.
Case Managers who are not licensed, registered, or certified by a health-related licensing board must complete 30 hours of continuing education every two years in serious mental illness and mental health services.
All clinical supervision must be documented in the child’s record .
Case Manager Associate (CMA) Eligibility (If Applicable)
Must be at least 21 years of age .
Must have a high school diploma or equivalent.
Must work under the direction of a qualified Case Manager or Case Management Supervisor.
May qualify with an associate degree in behavioral sciences/human services or qualifying experience as defined by statute.
Additionally, all candidates must:
Have experience and training in identifying and assessing a wide range of children’s needs.
Be knowledgeable about local community resources and how to access and coordinate those resources for the benefit of children and their families.
Meet all required supervision, training, and continuing education requirements as applicable.
Pay:
$24-$26 per hour
