Solution &ndash; Based casework helps children and families to be the solution to the seemingly insurmountable problems in their lives, from generational cycles of poverty, abuse, and neglect to unemployment, domestic violence, and severe health problems. By recognizing and reinforcing their innate strengths and ability to cope, we at Graham Windham empower them to take the lead in making change in their lives.</p><p><strong><u>About the Program</u></strong></p><p>At Graham Windham our Family Foster Care programs help children to get back to their families, adopted into a new family or connected to someone who is willing to be an anchor for children, throughout their childhood and into adulthood. To help us do that, we have hundreds of loving &amp; highly qualified foster parents primarily in Brooklyn, Manhattan, the Bronx and Westchester County- who serve as our foster children&#39;s substitute family.<br>&nbsp;&nbsp;<br><strong><u>About the Position</u></strong></p><p>The Intake Planner is responsible for the appropriate matching and safe placement of children entering regular and kinship foster homes; the replacement of children currently in foster care placements with Graham Windham; and the monitoring and maintenance of the Connections Vacancy Control and its functions.&nbsp;</p><p><strong><u>Duties and Responsibilities:</u></strong></p><ul type=\"disc\"><li>Intake Planner must liaise with the Intake Specialist when in doubt of making placements</li><li>Utilize Solution Based Casework practice in placement discussion with ACS Office of Placement Administration (OPA) to identify and match children with the appropriate family based the needs of children.&nbsp;</li><li>&nbsp;</li><li>Respond to internal requests for replacement in a timely manner and utilize SBC practice on the facilitation of replacements with case planners.&nbsp;</li><li>Assign all new referrals from OPA by using the vacancy control list.</li><li>&nbsp;Maintain and share accurate daily foster home vacancy control list with ACS Office of Placement Administration. &nbsp;</li><li>Respond to all ACS OPA after hour intake coverage rotation calls at night and weekends for the purpose of placement of children.</li><li>Prepare and distribute Intake admission form of all children accepted for placement from ACS with the case planning, medical and education teams.&nbsp;</li><li>Complete Birth Certificates, furniture and Day Care requests as required by Graham&rsquo;s protocols and policies.</li><li>Contact foster parents within 24 hours after each new child is placed in a foster home.&nbsp;</li><li>Attend all ACS Placement meetings.</li><li>Collaborate with the Fiscal team to ensure accurate processing of all new admissions. &nbsp;</li><li>Maintain record of all OPA referral placement acceptance and rejection.</li></ul><p><br></p><p><strong><u>Qualifications&nbsp;</u></strong>&nbsp; &nbsp;</p><p>BA/BSW Spanish speaking with up to 5 years of foster care experience required</p><p>Flexibility to work evenings and travel w/in the 5 boroughs and Westchester County, extended hours as necessary; Time Management and Organizational skills; Knowledge of MAPP and Mini MAPP, knowledge of internal and external regulations and polices applicable to foster care and adoption; Computer literate and familiar with CONNECTIONS and EVOLV; Bilingual Spanish/English and valid NYS driver&rsquo;s license a plus .Master&#39;s Degree in Social Work, Human Services or related field, conversational Spanish speaking with Min. 1 year Foster Care experience preferred.<br>&nbsp;&nbsp;</p><p><strong>Graham&#39;s Benefits</strong></p><p>Graham Windham offers eligible employees a comprehensive benefit package including medical, dental, and vision coverage, paid time off, holiday pay, retirement savings plan, and much more.</p><p>In addition to our comprehensive benefits package, Graham offers eligible employees a merit payment annually, based on performance evaluation. &nbsp;The merit is awarded in addition to the base salary. &nbsp;Employees typically receive an additional 3% - 6% of their annualize salary based on performance evaluation (this percentage is not guaranteed, however, the #s are based on merit awarded in prior years). &nbsp;</p><p>When you incorporate your base salary + the annual merit + our comprehensive benefits package which includes a pension plan, you will find that Graham&#39;s full compensation package is highly competitive. &nbsp;</p><p>Salary:&nbsp; $49,393</p><p><strong>&nbsp;EOE</strong></p><p><br></p><div><link href=\"https://cdn.jsdelivr.net/npm/[email protected]/css/froala_style.min.css\" rel=\"stylesheet\" type=\"text/css\"><div class=\"fr-view\"><div><p class=\"msonormal\"><br></p><p align=\"center\" class=\"msonormal\" style=\"text-align:center;\"><strong><span style='font-family:\"Franklin Gothic Book\",\"sans-serif\";'>EOE</span></strong></p></div></div></div></div></div></div></div></div>\n",
