    "ExternalDescriptionStr": "<p><strong>Job Description</strong></p>\n<p>&nbsp;</p>\n<p><strong>Job Summary</strong></p>\n<p><br>\n  The Specialist, Patient Care Delivery is a multi-disciplinary healthcare professional who delivers high-quality, compassionate care to health plan members in their homes. This role combines clinical expertise, cultural sensitivity, and excellent communication to ensure each member receives personalized and efficient care in a comfortable setting.</p>\n<p>&nbsp;</p>\n<p><strong>Job Duties</strong></p>\n<p><strong>In-Home Clinical Care</strong></p>\n<ul>\n <li>Travels to members’ homes to conduct scheduled health visits according to license-specific scope of practice. Drives up to 120 miles a day on a regular basis to a variety of locations within the assigned region. Drives beyond 120 miles as part of extended mileage may be required on special project days. Special projects may include an overnight hotel stay</li>\n <li>Collects and documents vital signs, medical history, and current medications in the secure electronic health record (EHR).</li>\n <li>Supports providers during in-home physical assessments and procedures (e.g., diabetic eye exams, bone density screens, vision/hearing screens).</li>\n <li>Administers injections, draws blood, and performs point-of-care testing (e.g., urine dipstick, glucose, pregnancy tests).</li>\n <li>Maintains inventory of mobile supplies and ensures safe handling, sterilization, and disposal of equipment and medical waste.</li>\n <li>Provides care and performs tasks aligned with credentialed role.</li>\n</ul>\n<p><strong>Member-Centered Engagement</strong></p>\n<ul>\n <li>Educates and guides members on care plans, medications, and healthy lifestyle practices.</li>\n <li>Communicates respectfully and effectively with diverse member populations, demonstrating cultural competence and emotional intelligence.</li>\n <li>Ensures member privacy and upholds HIPAA compliance within home-based environments.</li>\n <li>Builds trust to encourage member participation and long-term wellness engagement.</li>\n</ul>\n<p><strong>Mobile Administrative Support</strong></p>\n<ul>\n <li>Organizes and confirms in-home visit schedules and travel routes.</li>\n <li>Completes lab requisitions, coordinates specimen pickup, and manages referrals or follow-up needs.</li>\n <li>Accurately documents all care interactions and assists with coding or billing workflows as needed.</li>\n <li>Serves as a field liaison to the broader care team, identifying changes in member status and escalating appropriately.</li>\n</ul>\n<p>&nbsp;</p>\n<p><strong>Job Qualifications</strong></p>\n<p><strong>REQUIRED QUALIFICATIONS:</strong></p>\n<ul>\n <li>Active license or certification as one of the following: Medical Assistant (MA), Certified Nursing Assistant (CNA), Licensed Vocational/Practical Nurse (LVN/LPN), or Phlebotomist.</li>\n <li>Experience-based clinical proficiency aligned with credentialed role.</li>\n <li>Completion of training program applicable to credential.</li>\n <li>Current CPR certification.</li>\n <li>Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements.</li>\n <li>If not currently certified for phlebotomy, must obtain certification within 6 months of hire.</li>\n <li>Outstanding communication and interpersonal skills.</li>\n <li>Ability to work independently and adapt to varying home conditions.</li>\n <li>Knowledge of clinical safety, infection control, and emergency response.</li>\n <li>Comfort using technology for documentation, scheduling, and coordination.</li>\n <li>Commitment to continuous professional development and member-first care.</li>\n</ul>\n<p><strong>PREFERRED QUALIFICATIONS:</strong></p>\n<ul>\n <li>Bilingual (English/Spanish or other language relevant to member population).</li>\n <li>Experience with Medicaid/Medicare populations.</li>\n <li>Familiarity with mobile EHR systems (e.g., Epic, QNXT).</li>\n <li>Prior experience in home health or mobile care delivery.</li>\n</ul>\n<p>&nbsp;</p>\n<p>&nbsp;</p>\n<p><span>To all current Molina employees: If you are interested in applying for this position, please apply through the intranet job listing.</span><br><br><span>Molina Healthcare offers a competitive benefits and compensation package.
