</span></li>\n</ul>\n<ul>\n<li><span><span>Provides telephonic follow up with members</span><span> to provide medication reconciliation and medication management to assigned patients including transitional care and chronic </span><span>disease support.</span><span> </span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Complete assigned tasks related to medication management services.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>W</span><span>orking with </span><span>member</span><span>, </span><span>physician</span><span>, </span><span>health </span><span>system(s)</span><span>, pharmacy services, and </span><span>other </span><span>care providers to meet</span><span> the </span><span>member’s</span><span> individual needs based on </span></li>\n</ul>\n<ul>\n<li><span><span>P</span><span>rovides medication management services</span><span> </span><span>utilizing</span><span> prescribed </span><span>workflows.</span><span> </span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Complete assigned appointments </span><span>in accordance with</span><span> workflow</span><span>.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Assess patient adherence </span><span>behaviors.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Eliminate</span><span> barriers to </span><span>adherence.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Assess </span><span>appropriateness</span><span> of </span><span>therapy</span><span> provided by the </span><span>pharmacy.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Document appropriately in </span><span>Engage application and </span><span>EnterpriseRx</span><span> as goals </span><span>regarding</span><span> their specific chronic condition based on the Plan of Care</span><span>.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Support the evolution of pharmacy services delivery for key patient populations if applicable.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Al</span><span>l communications will be conducted in a professional manner. </span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>R</span><span>emain</span><span>s</span><span> </span><span>objective</span><span> </span><span>and unbiased decisions </span><span>in their </span><span>medication management </span><span>review</span><span> and recommendations</span><span> and </span><span>not</span><span> th</span><span>e member specific Plan of Care.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Completes clear and concise documentation in Care Management programs.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Complies</span><span> with</span><span> Customer Service expectations as applicable to the Department and Health Plan.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>TX state pharmacy license and/or assigned market </span><span>license.</span><span> </span></span><span> </span></li>\n<li><span>Residential state licensed in good standing</span></li>\n<li><span>Clinically required licenses in good standing</span></li>\n<li><span>PIC required licenses in good standing</span></li>\n</ul>\n<ul>\n<li><span><span>Strong patient engagement skills </span><span>including</span><span> active listening, de-escalation intervention </span><span>training</span><span> and ability to teach complex topics in an understandable manner.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Strong organizational and time management skills, able to work with little supervision while handling
