    "ExternalDescriptionStr": "<p>Inova Fairfax&nbsp;Hospital is looking for a dedicated Transplant Coordinator to join the Post-Lung Transplant team, providing management care for post-lung transplant patients. Responsible for aspects of care/communication related to assigned caseload of patients.</li><li>Educates patient, family, and care providers. Plans, implements, and evaluates formal and informal education regarding all aspects of care for the patient, family and caregivers and documents appropriately.</li><li>Organizes and submits data to ensure compliance with regulatory guidelines. Maintains entry, review and validation of all required data for caseload of patients according to protocol. Maintains the accuracy and timeliness of reporting with a greater than 95% compliance. Organizes and reports data to ensure efficient and consistent operation of the department within regulatory guidelines and for research and clinical care.</li><li>Actively participates in program quality assessment and performance improvement initiatives including committees or teams.</li><li>Participates in quality improvement initiatives and assists with process and protocol development/revision based on evidence based/best practice findings.</li><li>Collaborates effectively with the interdisciplinary team and leverages the expertise of the team to achieve patient outcomes.</li><li>Ensures improvements in practice by assuming responsibility for continuing education including obtaining and maintaining professional certification.</li><li>Performs other duties as assigned.</li></ul><p><strong>Minimum Qualifications:</strong></p><ul style=\"list-style-type: disc;\"><li><strong>Education</strong>: Bachelor’s degree in nursing</li><li><strong>Experience</strong>: Three years of RN experience</li><li><strong>Certification:&nbsp;</strong>Licensed in the Commonwealth of Virginia as a Registered Nurse; Basic Life Support from the American Heart Association</li></ul><p><strong>Preferred Qualifications</strong>:&nbsp;</p><ul style=\"list-style-type: disc;\"><li><strong>Experience</strong>: critical care/med-surg; advanced lung disease</li></ul><div>&nbsp;</div>",
