Transform Palliative Services. </strong></p>\n<p> </p>\n<p>We are seeking a compassionate and experienced Advanced Illness Management (AIM) Palliative Registered Nurse to join our team. Reporting to the Director of Palliative Care, this role provides high-quality palliative care to patients in their homes, assisted living facilities, and skilled nursing facilities. The AIM Palliative RN collaborates with patients, families, and interdisciplinary team members to assess, educate, and coordinate care while ensuring comfort and quality of life.</p>\n<p> </p>\n<p><strong>Essential Functions of Position:</strong></p>\n<ul>\n <li><p>Conduct in-person and telephonic RN assessments and screenings for palliative care patients.</p></li>\n <li><p>Complete Risk Stratification Assessments to identify patient support needs in consultation with the Nurse Practitioner.</p></li>\n <li><p>Observe patient visit frequency based on individual needs and risk identification.</p></li>\n <li><p>Educate patients and families on disease processes, self-care, end-of-life care, medications, nutrition, and treatments.</p></li>\n <li><p>Communicate findings to the Palliative Nurse Practitioner and/or PCP, including changes in condition or signs of decline.</p></li>\n <li><p>Coordinate care with palliative team members and support hospice referral as appropriate.</p></li>\n <li><p>Document timely clinical records including assessments, progress notes, care plans, physician orders, and treatments.</p></li>\n <li><p>Support care plan development from initial assessment through discharge, addressing functional decline, symptom management, nutrition, cognition, and family/caregiver needs.</p></li>\n <li><p>Collaborate on modifications to individualized palliative plans of care.</p></li>\n <li><p>Facilitate care delivery and obtain additional resources as needed in accordance with the Plan of Care.</p></li>\n <li><p>Participate in evening/weekend on-call rotation as required.</p></li>\n <li><p>Facilitate prescription transmission to pharmacies, manage lab and diagnostic results, and request durable medical equipment as medically necessary.</p></li>\n <li><p>Maintain effective relationships with supportive care team members and other healthcare providers.</p></li>\n <li><p>Communicate and document clinical findings, treatment plans, and care provided in accordance with nursing standards and regulatory requirements.</p></li>\n <li><p>Adhere to company policies, HIPAA privacy standards, business ethics, and mandatory compliance programs.</p></li>\n <li><p>Implement marketing and promotional initiatives as directed.</p></li>\n <li><p>Perform other related duties as assigned.</p></li>\n</ul>\n<h2>About You</h2>\n<h3><strong>Required Qualifications:</strong></h3>\n<ul>\n <li>Active and unrestricted Registered Nurse (RN) license with no board actions</li>\n <li>Minimum 2 years of clinical RN experience, preferably in skilled nursing, home health, palliative care, hospice, geriatrics, or oncology</li>\n <li>Demonstrated ability to provide quality care in high-empathy environments</li>\n <li>Proficiency in Microsoft Outlook, Word, Excel, and electronic medical records</li>\n <li>Exceptional communication, time management, and critical thinking skills</li>\n <li>Keen observation skills with the ability to identify changes in condition and escalate appropriately</li>\n <li>Comfortable working independently in the field and within interdisciplinary care models</li>\n <li>Valid driver’s license and reliable transportation (field-based role)</li>\n</ul>\n<p> </p>\n<h3><strong>Preferred Experience:</strong></h3>\n<ul>\n <li>Previous experience in palliative care, hospice, or oncology nursing</li>\n <li>Familiarity with risk stratification, symptom management, and home-based care planning</li>\n <li>Experience working in or with assisted living and skilled nursing facilities</li>\n</ul>\n<h2>We Offer</h2>\n<p><strong>Benefits for All Associates (Full-Time, Part-Time & Per Diem):</strong></p>\n<ul>\n <li>Competitive Pay</li>\n <li>401(k) with Company Match</li>\n <li>Career Advancement Opportunities</li>\n <li>National & Local Recognition Programs</li>\n <li>Teammate Assistance Fund</li>\n</ul>\n<p> </p>\n<p><strong>Additional Full-Time Benefits:</strong></p>\n<ul>\n <li>Medical, Dental, Vision Insurance</li>\n <li>Mileage Reimbursement or Fleet Vehicle Program</li>\n <li>Generous Paid Time Off + 7 Paid Holidays</li>\n <li>Wellness Programs (Telemedicine, Diabetes Management, Joint & Spine Concierge Care)</li>\n <li>Education Support & Tuition Assistance (ASN to BSN, BSN to MSN)</li>\n <li>Free Continuing Education Units (CEUs)</li>\n <li>Company-paid Life & Long-Term Disability Insurance</li>\n <li>Voluntary Benefits (Pet, Critical Illness, Accident, LTC)</li>\n</ul>\n<h2>Legalese</h2>\n<ul>\n <li>This is a safety-sensitive position</li>\n <li>Employee must meet minimum requirements to be eligible for benefits</li>\n <li>Where applicable, employee must meet state specific requirements</li>\n <li>We are proud to be an EEO employer</li>\n <li>We maintain a drug-free workplace</li>\n</ul>\n<h2>Location</h2>Illumia\n<h2>Our Company</h2>\n<p> </p>\n<p></p>\n<p> </p>\n<p>At Illumia, part of Gentiva, it is our privilege to offer compassionate care in the comfort of wherever our patients call home.
