Description Job Function Summary: Involves the provision of integrated and accessible primary and specialty patient care services in a clinic or center environment. Care is provided by clinicians and assistants who are accountable for addressing a large majority of personal health care needs and developing a sustainable partnership with patients. Ensures optimal patient outcomes through effective leadership and / or provision of patient care delivery in healthcare clinics or centers. Promote shared decision-making at all levels to ensure a shared voice in practice, exchange of ideas, enhanced communication, and continually improved patient services. Management levels may also be responsible for clinic operations. (Note, when primary focus is clinical operations, see Ambulatory Administration job function.) Custom Scope ) : The Nurse Supervisor (NS) is part of a dynamic care team within the Office of Population Health (OPH) at UCSF Health. This position is a mission-driven nurse leader that will bring a leadership presence, sound judgment, and a commitment to operational excellence in advancing innovative models of care. This NS provides operational clinical oversight for the following disciplines: Registered Nurses, Advance Nurse Specialists, other Nursing positions who work in the areas of Complex Care Management, Transitions, and other Care Coordination Programs for both Adults and Pediatrics. In addition will participate in the multiple OPH Community Events. This position will serve as a clinical leader resource and liaison for all staff within the areas they report to. The position will be responsible for overall operations in addition to the achievement of outcomes. In addition, the Supervisor will ensure compliance around all compliance and regulatory requirements. Will participate in any current or new accreditation obtained by their teams. Will be required to attend training program for the Enhanced Care Management (ECM) programs. The Supervisor will be responsible for working with providers to ensure that orders and referrals are carried out in a timely manner and ensure that any medical or psychological issues identified by the frontline team are escalated and addressed. This will be done through clinical protocols established by the OPH Leadership and Medical Directors and/or department liaisons. This position will report to the Assistant Clinical Director – Nursing. Will be part of the OPH leadership team and will collaborates closely with other OPH leaders including the Vice President Population Health, Chief Population Health Officer, Medical Directors, other Managers and Director and key internal UCSFH Health and external community partners. This is an on-site leadership position requiring a four-day-per-week presence or as directed by the telework agreement. Will need to have flexibility to work across UCSF Health locations (East and West Bay). Responsibilities % of time Essential Function (Yes/No) Key Responsibilities (To be completed by Supervisor) 30 Yes Leadership & Operations Provide operational leadership across assigned programs, ensuring that the flow of patient care activities for all assigned areas and coordination of the nursing teams who are part of the larger multidisciplinary team. Leads program development and execution of Standard Operating Procedures (SOPs) to ensure compliance around patient care. Partner with the Assistant Clinical Director and other OPH leadership to address operational challenges and implement improvements. Foster a high-performance culture by setting clear expectations and motivating teams. May be a primary or delegate for other UCSF Health meetings, collaboratives, and will also partner with community organizations to ensure that all external resources are used for patient referrals. 20 Yes People Leadership & Human Resources Collaborates with supervisor to oversee workforce management, including hiring, onboarding, training, and performance management. Advanced knowledge with HR policies, labor laws, and union requirements. Promote staff engagement, development, individual development and retention through coaching and mentorship. Collaborate with supervisor around employee relations issues and facilitate effective conflict resolution. 20 Yes Program Development & Implementation Participate in the development and implementation of standard operating procedures (SOPs), care pathways, and workflows. Design and execute implementation plans in collaboration with stakeholders. Apply performance improvement methodologies (e.g., PDSA cycles) to drive innovation and continuous improvement. Develop tools, training, and resources to support program success. 10 Yes Financial & Resource Management Works with supervisor to review annual budgets and align with monthly financial performance. Ensure efficient resource utilization and productivity targets are met. 10 Yes Quality, Evaluation & Performance Improvement Accountable for the monitoring of key performance metrics. Participate in data-driven decision-making and continuous evaluation of program outcomes. Identify opportunities for improvement and implement evidence-based interventions. Identify risks and benefits of program changes to ensure achievement of targets and high-quality care delivery. 5 Yes Partnerships, Communication & Reporting Build and maintain partnerships with internal departments, providers, and community-based organizations. Collaborate to address barriers to care, improve access, and reduce health disparities. Assist in the creation of reports, presentations, and updates to internal and external stakeholders Participate in local, state, and national collaboratives to advance population health initiatives 5 Yes Other duties as assigned . Qualifications Required qualifications: Requires 1 year or more of Lead/Charge/Supervisory/Manager experience in a hospital, clinic, or care management-based setting Should have at least 5 years working as a Registered Nurse (RN)in areas such as ambulatory, inpatient, public health or a Care Management setting. Demonstrated skills to effectively supervise clinicians and to manage the complex workflow and multiple priorities involved with a clinic(s) / center(s). In depth knowledge of patient care Standard Operating Procedures (SOPs) and care delivery, systems, external regulations, and internal policies. Strong interpersonal skills for effective collaborations and problem solving with all levels of management, staff, consultants, and outside agencies. Strong written and verbal communications skills, with the ability to train and mentor subordinates, and to convey complex clinical and technical information in a clear and concise manner. Preferred qualifications: Proficient in all software applications (Word, Excel, and PowerPoint) and database applications. Strong knowledge of human resources management policies, with the ability to train, monitor, evaluate, and document human resources issues and performance, and to participate in decision-making on human resources matters. Knowledge of analytical and critical thinking skills, with the ability to quickly analyze problems, determine appropriate level of intervention, and develop and apply effective solutions.
