Pacific Grove Hospital is proud to be a part of the Acadia Healthcare family.</p>\n<p> </p>\n<p>This position will be supporting our <strong>Outpatient Services</strong>.</p>\n<p> </p>\n<p>We are proud to offer a Competitive salary, Employee Assistance Program, and many learning opportunities.</p>\n<p> </p>\n<p>Hourly pay scale: $36-40 DOE</p>\n<p> </p>\n<p><strong>PURPOSE STATEMENT:</strong></p>\n<p>Plan, direct, define and coordinate individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance use and physical and emotional abuse.</p>\n<h2>Responsibilities</h2>\n<p><strong>ESSENTIAL FUNCTIONS:</strong></p>\n<ul>\n <li>Conduct psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times.</li>\n <li>Through patient observation and patient/family interview obtaining information on the patient’s needs, skills, interests and limitations for the development and implementation of the treatment plan.</li>\n <li>Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle. </li>\n <li>Participate in multidisciplinary treatment team meeting to discuss psychosocial assessment, treatment planning and ongoing discharge planning.</li>\n <li>Identify, assess and coordinate patient cases and anticipate their needs related to illness, hospitalization, finances, education and home/community services.</li>\n <li>Facilitate education, process groups, support groups and referrals as requested.</li>\n <li>May conduct intakes and assist in coordinating placements within facility programs.</li>\n <li>Promote recovery, symptom reduction and the achievement of the highest level of social functioning.</li>\n <li>Communicate or aid family members in understanding and supporting the patient.</li>\n <li>Manage a caseload and assume responsibility for patient chart documentation.</li>\n <li>May facilitate education, process groups, support groups and referrals as requested.</li>\n <li>Manage a caseload and maintain accurate and timely documentation to assure patient records comply with facility and accreditation standards.</li>\n <li>Demonstrate a positive, empathetic and professional attitude towards customers always.  When patient needs are not met, acknowledge and work to resolve complaints.
