Description Position Summary: Working as part of a collaborative care team, the person in this position will assist individuals with identify challenges to sustained healthy parenting to obtain health, residential, financial and social services resources to assist in supporting identified needs, provide needs assessments and develop and participate in care management plan development and implementation. Will advocate on the individual’s behalf for services needed, link with other agencies and internally for services, and refer as needed. Participates in treatment plans and provides advisory and consultative services related to programming. Mobile MAT program is a healthcare service that delivers addiction treatment directly to individuals in their communities using mobile units, such as vans or specialized vehicles. These programs provide medication assisted treatment, which typically includes medications combined with counseling and support services, to help individuals with opioid use disorder manage cravings, reduce withdrawal symptoms, and support long-term recovery. These units are designed to reach underserved or remote areas, offering greater accessibility to individuals who may face barriers like transportation issues, limited access to healthcare facilities, or social stigma. In addition to providing medication, the mobile MAT program offers a range of supportive services, such as mental health counseling, case management, risk reduction education, and referrals to additional healthcare or social services. By bringing these resources directly to communities in need, mobile MAT program aim to increase access to comprehensive addiction treatment, improve patient outcomes, and reduce overdose deaths. Essential Job Functions: Assess need and make decisions regarding the delivery of the appropriate individual, group and family supportive services both internal and external. Make assessments of potential clients to determine need and appropriateness for program participation and referrals. Analyze situations and take necessary actions to provide crisis intervention.  Interpreting and implementing WestCare procedures regarding crisis intervention as appropriate. Monitor and assess client behavior. Provide input into Individual Treatment/Service Plans. Report to immediate supervisor any unusual or reportable incidents that should be brought to the attention of the Program Manager, or regulatory agency. Participate in decision to refer clients for ancillary services, i.e., family support groups, continuing care groups, etc. Attend all interdisciplinary treatment team meetings and establish parameters for discharge planning. Collaborate and engage with community resources to proactively prevent domestic violence, child welfare system involvement and promote sustained healthy relationships by addressing immediate needs of all program participants. Educate and refer individuals to community agencies and businesses that appropriately address their needs for food, shelter, clothing, employment, transportation, etc. Assist with establishing or re-establishing family and community support systems Act as advocate to improve access to needed services Participate in clinical staffing decisions regarding the individual’s current strengths and resources and their need for additional resources Complete documentation of services in a timely manner in the electronic health record Provide input into the patient’s treatment needs and disposition Assists other staff as needed in a collaborative manner to promote cooperation and portray a professional image to the community. Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation Qualifications Essential Qualifications: Certifications/Licensures: Certification as a Certified Case Manager (CBHCM or CWCM) in good standing by an agency approved and recognized by the Florida Certification Board is preferred at time of hire; Preference will be given to those candidates who are currently Certified; Education: Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required Experience and Competencies: A minimum two (2) years of related experience providing case management and/or care coordination services preferred. Bilingual preferred. Valid driver’s license and ability to obtain and maintain Must have reliable transportation Must have availability to work a flexible schedule including evenings and weekends. Maintains confidential files and records. Ability to identify and describe key signs, symptoms, impact and manifestations of trauma Ability to prioritize and organize competing consumer and family needs Ability to use trauma informed care principles including ensuring consumer safety, preventing/avoiding re-traumatization, maximizing trustworthiness, enhancing consumer choice, maximizing collaboration, and emphasizing empowerment Aware of the differences between trauma informed and trauma-specific services Knowledge and application of psychopathology theory Knowledge in the understanding of Motivational Interviewing; Motivational Enhancement Therapy; Comprehensive, Continuous Integrated System of Care (CCISC); and Trauma-Informed Care Knowledge of the inter-relationship between substance abuse and mental health disorders The ability to assess the individual’s growth and development relative to normative development for his/her chronological age, including physical, emotional, cognitive, educational, and social development The ability to conceptualize cases within a developmental framework for the purpose of service planning, therapeutic intervention, and discharge planning The ability to coordinate and integrate multiple services, as well as interface and communicate with internal and external providers The ability to counsel individuals and families in the achievement of objectives through the exploration of a problem and its ramifications; the examination of attitudes and feelings; the consideration of alternative solutions; and decision-making The ability to determine eligibility of a consumer’s admission The ability to identify and evaluate an individual’s strengths, barriers, problems, and needs for the development of a Service Plan The ability to identify crises when they surface; mitigate or resolve the immediate problem; and de-escalate the situation, including the ability to develop Safety Plans, as needed The ability to identify the needs of a consumer and family that cannot be met by the agency and to assist the consumer/family to utilize the support systems and community resources available The ability to provide information to individuals and families concerning alcohol or other drug use, mental health problems, and the available services and resources The ability to provide service planning through the identification and prioritization of problems needing resolution; the development of goals and objectives for the consumer which are expressed in behavioral and measurable terms; and the ability to negotiate goals and objectives with the consumer and the family Understanding and application of federal, state, local and agency confidentiality rules and professional standards of conduct Understanding of family dynamics, family roles, and family structure Knowledge of WestCare Mission, goals and values Knowledge of Incident Reporting Knowledge of HIPAA, Professional Ethics, Consumer Rights and Confidentiality Familiarity with WestCare services and programs Demonstrates a high degree of dependability. Maintains a good attendance record. Ability utilize Microsoft Office Software Suite Ability to provide an Affidavit of Good Moral Character Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required. Working Conditions: Work is performed in multiple community based settings including the individual’s place of residence. A minimum of 40 hours per week are expected but significantly more hours will be required from time to time.  Completion of job duties may require working before and after normal working hours.  Regular attendance is required.  Being able to work as many hours as is necessary to complete job tasks is a required and essential duty of this position. Appropriate personal protective equipment will be readily available to every employee.
