The per diem Registered Nurse provides clinical expertise that supports quality improvement interventions and improved health outcomes. Such support operates under a whole-person approach to delivering care, integrating behavioral health, physical health, and developmental disability services, as well as other necessary supports and social services.</p>\n<p>At Partnership Solutions, our work is performed in support of and on behalf of Care Design NY. Click here to view the current benefits summary.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Support healthcare initiatives in providing clinical support to complex people with I/DD.</li>\n <li>Work collaboratively with the Nurse Manager of Healthcare Management to risk-stratify individuals for clinical assignments.</li>\n <li>Carry a clinical assignment to support care managers with individuals who present as highly clinically complex.</li>\n <li>Perform electronic chart reviews with a focus on quality improvement and continuity of care.</li>\n <li>Documents clinical findings and recommendations in the electronic health record within the scope of their license.</li>\n <li>Provide clinical guidance to support care managers in coordinating and accessing preventative and health promotion services.</li>\n <li>Participate in complex care case reviews, clinical rounds, and other teams as needed to problem solve and assist those members most in need of a comprehensive and creative approach.</li>\n <li>Meet all training requirements on time.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>A Registered Nurse license to practice in New York state required.</li>\n <li>A BSN is preferred.</li>\n <li>Previous chart review experience required.</li>\n <li>A minimum of 3 years of clinical experience in a hospital, skilled nursing facility, or subacute rehabilitation center.</li>\n <li>Personal or professional I/DD experience is strongly preferred.</li>\n <li>Experience in a managed care plan or health home is preferred.</li>\n <li>Previous quality improvement experience preferred.</li>\n</ul>\n<h2>Salary starting at</h2>$40 / hour\n<h2>Salary up to</h2>$50 / hour",
