You’re looking in the right place.</p>\n<p> </p>\n<p>As a <strong>Speech-Language Pathologist</strong>, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners. </p>\n<p> </p>\n<p>Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.</p>\n<p> </p>\n<p>As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness and work-life balance.</p>\n<p> </p>\n<p><strong>What will you do ?</strong></p>\n<p>Reporting to the Manager, Patient Services, the Speech Language Pathologist provides speech language therapy services to ONTARIO HEALTH ATHOME Champlain clients in homes, schools, and nursing homes through clinical assessment, treatment plans, goals, consultation with family and other interdisciplinary team members. The Speech Language Pathologist provides recommendations for follow-up and management of the client’s condition.</p>\n<p> </p>\n<p><strong>Primary Responsibilities:</strong></p>\n<ul>\n <li>Responsible for assessing, implementing treatment, setting goals, and ongoing evaluation of speech therapy treatment plans for ONTARIO HEALTH ATHOME Champlain patients.</li>\n <li>Responsible for scheduling treatment visits to meet patient priorities.</li>\n <li>Responsible for keeping the Care Coordinator informed of the client’s progress, through verbal and written reports.</li>\n <li>Functions as a member of a multidisciplinary team involved in ongoing communication concerning the patient.</li>\n <li>Maintains client records and provides reports according to ONTARIO HEALTH ATHOME Champlain policies.</li>\n <li>Responsible for teaching client, family and other team members, the treatment program and management.</li>\n <li>Acts as a resource person and consultant for long term care homes and provides therapy to residents who meet ONTARIO HEALTH ATHOME Champlain admission criteria.</li>\n <li>Maintains supplies and recommends equipment for department use within budget limitations.</li>\n <li>Acts as a resource person for other health professionals.</li>\n <li>Participates in committees as approved.</li>\n <li>Interprets the role and objectives of the LHIN to the community and interested agencies.</li>\n <li>Partakes in professional development activities and maintains knowledge in the professional field</li>\n</ul>\n<p> </p>\n<p><strong>Team/Department: </strong>Stroke Rehab</p>\n<p><strong>Hours of work: </strong>8:30am-4:30pm – Monday to Friday</p>\n<p><strong>FTE:</strong> 1.0</p>\n<p><strong>Length of Temporary Assignment: </strong>September 7, 2026</p>\n<p><strong>Starting Salary: </strong>$47.259 - $50.689 /hr</p>\n<p><strong>Affiliation: </strong>ONA</p>\n<p><strong>Reporting to:</strong> Manager Patient Services </p>\n<p><strong>Office Location:</strong>  Bells Corners</p>\n<p> </p>\n<p><strong>What must you have?</strong></p>\n<ul>\n <li>Master’s degree in Speech Therapy and current certificate of registration with the College of Audiologists and Speech-Language Pathologists of Ontario</li>\n <li>Related professional experience which would normally be attained over a period of two years in a SLP role</li>\n <li>Related experience in a similar community-based setting is an asset</li>\n <li>We have a mandatory COVID-19 vaccination policy.
