text-align: justify;\"><span style=\"font-size: 12pt; line-height: 107%;\">Schedule: Monday - Friday 8:30am-5:00pm (1 hour lunch) (FT)</span><span>.&nbsp;</span><span style=\"font-size: 12pt; line-height: 107%;\">Weekly Hours: 37.5</span></p></li><li class=\"ortl-align-justify\"><p style=\"line-height: 115%; text-align: justify;\"><span style=\"font-size: 12pt; line-height: 107%;\">Current active and unrestricted license and registration in New York State as a Registered Nurse required.</span></p></li><li><span style=\"font-size: 12pt; line-height: 107%;\">Minimum of two (2+) years of administrative experience in a management</span><span>&nbsp;</span><span style=\"font-size: 12pt; line-height: 107%;\">capacity in a certified home health agency (CHHA) and/or long-term home</span><span>&nbsp;</span><span style=\"font-size: 12pt; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Oversee development and execution of individualized care plans to improve satisfaction and outcomes.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Lead and document interdisciplinary team meetings; complete attestations; support Virtual interdisciplinary team.</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Serve as manager on call after hours; complete follow-ups, documentation, and initiate grievances promptly.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Run Clinical Delivery reports.</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Allocate staffing and resources to ensure seamless coverage and operations.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Coordinate ongoing communication with IDT and stakeholders to align care with participant goals.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Review UAS/UAS analyzer with clinicians to ensure assessment accuracy.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Investigate incidents and complete Level 2 reviews and documentation per regulations.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Ensure complete, accurate participant documentation (enrollment, disenrollment, EMR, clinical notes) and perform chart audits.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Coordinate transitions of care and arrange long-term care services as needed.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Monitor care management effectiveness/utilization and adjust plans accordingly; ensure compliance with organizational, state, and federal standards.&nbsp;</span></p></li><li><p style=\"background-color: white; line-height: normal; margin-bottom: 0in;\"><span style=\"color: black;\">Drive quality improvement; stay current on clinical guidelines and practice standards.&nbsp;</span></p></li><li><p style=\"background-color: white;
